军队高龄男性高尿酸血症与代谢综合征的相关性研究

摘　　要：目的探讨军队高龄男性老人高尿酸血症(HUA)的临床特点及其与代谢综合征的相关性。方法收集军队干休所参加健康查体的416名高龄老人(≥80岁)的临床资料,对血尿酸及相关代谢因素进行横断面分析。结果在资料完整的383名高龄男性中,HUA的患病率为24.3%,HUA组合并肾功能不全的比例明显高于血尿酸正常组(33.3%vs 20.3%,P<0.05),HUA组合并代谢综合征的比例远高于血尿酸正常组(67.7%vs 39.7%,P<0.05),血尿酸水平与腰围、三酰甘油水平呈显著正相关(r值分别为0.220和0.113,P<0.05),与高密度脂蛋白水平呈显著负相关(r=-0.150,P<0.05),与低密度脂蛋白、血压(包括收缩压和舒张压)及空腹血糖水平无明显相关性(P>0.05)。血尿酸升高是代谢综合征发生的危险因素,HUA患者发生代谢综合征的风险是血尿酸正常者的3.196倍(OR=3.196,P=0.000)。结论军队高龄男性老人HUA的患病率较高,多合并肾功能不全,且常伴随肥胖、脂代谢紊乱等代谢疾病,血尿酸升高与代谢综合征的发生密切相关。Objective To investigate the relationship between hyperuricemia（HUA） with metabolic syndrome（MS） and its components among the very elderly. Methods The cross-sectional study was based on 416 senile subjects（aged ≥ 80 years） from a military retirement community with detailed information were collected. Results Eligible data of 383 male subjects were enrolled in this study. The prevalence of HUA was 24.3%. Significant more patients were complicated with renal insufficiency in HUA group than in non-HUA group（33.3% vs 20.3%,P〈0.05）,Far more patients were accompanied with MS in HUA group than in nonHUA group（67.7% vs 39.7%,P〈0.05）. Uric acid concentration was significantly positively correlated with waist circumstance and triglyceride（r = 0.220 and 0.113,respectively,P〈0.05）. Uric acid concentration was statistically negatively correlated with highdensity lipoprotein-cholesterol（r =-0.150,P〈0.05）. There was no correlation between uric acid concentration with low-density lipoprotein-cholesterol, blood pressure and fast plasma glucose（P〉0.05）. Multiple logistic regression analysis showed that HUA was a risk factor of MS.The senile males with HUA had a 3.196-fold increased risk of MS as compared with those without HUA（OR =3.196,P = 0.000）. Conclusion HUA is highly prevalent among the military very elderly. The higher uric acid concentration is frequently complicated with renal insufficiency,and often accompanied with obesity and lipid metabolic disorders,which is positively associated with MS.
