摘 要:目的探讨三维斑点追踪成像(3D-STI)技术评价左心室不同构型尿毒症患者左心室整体收缩功能的价值。方法 57例尿毒症患者按照左心室不同构型分为左心室构型正常组(LVN组,27例)和左心室肥厚组(LVH组,30例),选取31例健康志愿者作为对照。采用3D-STI技术获取各组左心室整体纵向收缩期峰值应变(GLS)、左心室整体径向收缩期峰值应变(GRS)、左心室整体圆周收缩期峰值应变(GCS)、左心室整体面积收缩期峰值应变(GAS),比较三组指标间的差异。结果与正常对照组比较,LVN组GLS减低(P<0.05),GCS、GRS、GAS无明显变化(P>0.05),LVH组GLS、GCS、GRS、GAS均减低(P<0.05);与LVN组比较,LVH组GLS、GAS、GRS均减低(P<0.05)。结论 3D-STI技术可以早期、敏感地反映尿毒症患者左心室收缩功能的改变,为临床治疗提供参考依据。Objective To investigate the value of three‐dimensional speckle tracking imaging (3D‐STI) in evaluating sys‐tolic function in patients with uremia of different left ventricular configurations .Methods Fifty‐seven patients with uremia were divided according to different configurations of the left ventricular including left ventricular normal group (LVN group ,n = 27) and left ventricular hypertrophy group (LVH group ,n = 30) .Thirty‐one healthy volunteers were selected as controls .3D‐STI was used to obtain global left ventricular longitudinal systolic peak strain (GLS) ,global radial systolic peak strain (GRS) ,global circumferential systolic peak systolic strain (GCS) ,global area systolic peak strain (GAS) .In‐dicators were compared between the three groups .Results Compared with the control group ,GLS significantly decreased in LVN group ( P 〈 0 .05) ,GCS ,GRS and GAS showed no significant change ( P〉0 .05) .GLS ,GCS ,GRS and GAS of LVH group decreased ( P 〈 0 .05) .Compared with LVN group ,GLS ,GRS and GAS decreased in LVH group ( P 〈0.05) .Conclusion 3D‐STI technique can early ,sensitively reflect the change of left ventricular systolic function in pa‐tients with uremia ,and provide a reference for clinical treatment .
