摘 要:目的 探讨运用颅内电极埋藏技术,进行视频脑电监测,定位疑似岛叶癫痫的临床应用.方法 15例疑似岛叶癫痫综合征的患者,在岛叶、岛盖以及其他可疑位置,植入条状电极、栅状电极及深部电极,进行长程视频脑电监测,记录发作间期和发作期脑电图变化,明确致痫灶起始区,通过手术切除致痫灶.结果 本组15例颅内电极埋藏时间为5~21 d,全部记录到致痫灶发作间期和发作期癫痫样放电.确诊岛叶癫痫5例,颞叶癫痫5例,额叶癫痫3例,顶叶癫痫2例.本组均行致痫灶切除术.所有病例均未出现颅内埋藏电极引起的并发症.结论 在疑似岛叶癫痫综合征,采用颅内电极埋藏进行脑电监测,可以准确定位致痫灶,提高治愈率.Objective To investigate the application of intracranial buried electrode technique for video electroencephalogram(EEG) monitoring and to locate the suspected insular epilepsy in clinical application.Methods The strip,grid,and depth electrodes were implanted into insula,operculum,and other suspicious positions,and 15 patients with suspected insular epilepsy syndrome were monitored by long-term video EEG.The EEG changes of the interictal and ictal phases were recorded,and the origins of epileptic foci were identified and the epileptic foci were resected by surgery.Results The buried time of intracranial electrodes in 15 patients ranged from 5 to 21 days.The epileptogenic foci interictal period and ictal epileptiform discharges were all recorded.Five patients were diagnosed as insular epilepsy,5 were temporal lobe epilepsy,3 were frontal lobe epilepsy,and 2 were occipital lobe epilepsy.The patients accepted epileptogenic foci resection.All patients did not have any intracranial buried electrodes caused complications.Conclusion Using the intracranial buried electrodes for EEG monitoring,the epileptic foci of suspected insular epilepsy syndrome can be localized accurately and improve the cure rate.
