七氟醚复合瑞芬太尼在小儿扁桃体手术中的临床价值

摘  要:目的:探讨七氟醚联合瑞芬太尼在小儿扁桃体手术中的临床价值。方法:将我院78例应用扁桃体切除手术的小儿随机分为七氟醚组和丙泊酚组,记录两组患儿麻醉后5分钟、气管插管后5分钟、手术后5分钟及拔管后5分钟时的HR、BP及SpO2,并观察两组患儿在手术结束后自行呼吸恢复、主观意识恢复和气管拔管的时间。结果:七氟醚组在气管插管后5分钟、手术后5分钟HR、收缩压及舒张压显著高于丙泊酚组(P<0.05),在拔管后5分钟HR及收缩压显著高于丙泊酚组(P<0.05);在手术结束后自行呼吸恢复时间、主观意识恢复时间和气管拔管时间上较丙泊酚组明显缩短,有统计学意义(P<0.05)。结论:七氟醚复合瑞芬太尼麻醉在小儿扁桃体切除手术中,麻醉效果好、恢复速度快及血流动力平稳,不良反应少,值得推广。Objective:Discussion sevoflurane combined with remifentanil in pediatric clinical value of tonsilectomy.Method:78 cases of pediatric tonsillectomy surgery were randomly divided into sevoflurane group and propofol group, Records the HR、Bp and SpO2 of two groups of children 5 minutes after anesthesia、5 minutes after intubation、5 minutes after the operation and 5 minutes after extubation, observe two groups of children at the end of the operation to rescue breathing subjective consciousness recovery and tracheal extubation time.Results:Sevoflurane group 5 minutes after the operation,5 minutes after endotracheal intubation HR、systolic and diastolic blood pressure is significantly higher than the propofol group(P〈0.05), five minutes after extubation HR and systolic blood pressure was significantly higher than the propofol group(P〈0.05);At the end of the surgery itself breathing recovery time subjective consciousness recovery time and tracheal extubation time significantly shortened in the propofol group, there was statisticaly significant(P〈0.05).Conclusion:Sevoflurane and remifentanil anesthesia in pediatric tonsilectomy surgery, anesthesia effect is good、fast recovery speed and stable hemodynamic, fewer adverse reactions, worthy of popularization.
