护理干预对老年患者留置导尿漏尿及其并发症的影响

摘　　要：目的探讨早期护理干预对老年留置导尿患者漏尿发生率及其并发症的影响。方法将2013年4-12月60岁以上的65例留置尿管的患者作为对照组,将2014年1-9月60岁以上的65例留置导尿患者作为干预组。对照组男性患者使用16号导尿管,尿管气囊注水量15mL,女性患者使用18号导尿管,尿管气囊注水量20mL,保持尿液澄清并引流通畅。干预组男性患者使用14号导尿管,尿管气囊注水量10mL,女性患者使用16号导尿管,尿管气囊注水量15mL,两组患者均保持尿液澄清并引流通畅。观察比较两组患者漏尿发生率、患者舒适度、失禁相关性皮炎(IAD)及导尿管相关性尿路感染(CAUTI)发生率。结果干预组患者漏尿发生率、IAD及CAUTI发生率均低于对照组(P<0.01),而舒适度高于对照组(P<0.01)。结论早期护理干预能有效降低漏尿、IAD及CAUTI发生率,提高患者舒适度及护理质量。Objective To analyze the effect of early nursing intervention on incidence of urinary leakage and its complications for patients with indwelling catheter. Method 65 cases patients with urinary catheter from April 2013 to December 2013 was selected as control group. 65 cases of patients with indwelling catheter from January 2014 to September 2014 was selected as intervention group. Male patients with 16 catheter and airbag injection 15ml in control group, Female patients with 18 catheter and airbag injection 20 ml. All the patients were keep clear urine and urine tube drainage unblocked. In intervention group, male patients with 14 catheter and airbag injection 10 ml, female patients with 16 and airbag injection 15 ml. All the patients were keep clear urine and urine tube drainage unblocked. The incidence of urine leakage, patient comfort, incontinent dermatitis （IAD） and the correlation catheter associated urinary tract infection （CAUTI） between two group were analyzed. Result The incidence of urine leakage, IAD and the incidence of CAUTI in intervention group are much lower than that of the control group （P〈0.01）, and the comfort level is higher than that of control group （P〈0.01）. Conclusion Early nursing intervention can effectively reduce the incidence of urine leakage, improve patient comfort, reduce the incidence of IAD and CAUTI, improve the quality of nursing.
