原发性肝癌术后低体温患者个体因素对复温速度的影响

摘　　要：目的探讨在复温措施和治疗方案相同的情况下,原发性肝癌术后低体温患者自身因素对复温速度的影响。方法回顾性分析120例原发性肝癌术后低体温患者91例的复温过程,对可能影响体温恢复的个体因素,包括性别、年龄、身高、体质量、体重指数、体表面积、体表面积/体质量、术中出血量、手术时间、肿瘤直径等因素进行单因素分析,选取有统计学意义的因素进行多因素Logistic回归分析。结果单因素分析显示,体质量、体重指数、体表面积、体表面积/体重、肿瘤直径有统计学意义(P<0.05),进一步多因素Logistic回归分析表明,患者的体重指数与肿瘤直径是影响复温速度的个体因素。结论患者的体重指数和肿瘤大小可影响原发性肝癌术后低体温患者的复温速度,提示临床应加强对低体重指数及大肝癌术后低体温患者的复温护理,并根据患者的体重指数和肿瘤直径选择个性化的复温措施,提高复温效果。Objective To evaluate the effects of individual factors on the rewarming of primary liver cancer patients with low temperature after surgery treated by the same rewarming measures and treatment scheme. Method 91 primary liver cancer patients with hypothermic after surgery were analyzed retrospectively. Gender, age, height, weight, body mass index, body surface area, body surface area / weight, intraoperative blood loss, operative time and tumor diameter factors were processed in single factor analysis. The statistically significant factors was choose for multiple factors analysis. Result The single factor analysis showed that weight, body mass index, body surface area and tumor diameter were statistically significant （P〈0.05）, and the multiple factors analysis showed that body mass index and tumor diameter were the influences on the rewarming of hypothermic patients with primary hepatic carcinoma. Conclusion The body mass index and tumor diameter can affect the core temperature rising speed in patients with hepatic carcinoma. We should select personalized rewarming measures according to the patient＇s body mass index and tumor diameter, especially for the patients with low body mass index and large hepatic carcinoma.
