半腱肌解剖重建治疗膝关节内侧副韧带损伤的临床观察

摘　　要：目的:探讨半腱肌解剖重建治疗膝关节内侧副韧带(MCL)损伤的临床效果。方法:对58例膝关节内侧副韧带Ⅲ度损伤患者进行手术治疗,采用膝内侧副韧带修补,内侧副韧带浅层重建及后斜韧带部分重建方法。结果:治疗前,优秀4例(6.90%),良好15例(25.86%),尚可17例(29.31%),较差22例(37.93%),优良率62.07%;治疗后,优秀24例(41.38%),良好19例(32.76%),尚可9例(15.52%),较差6例(10.34%),优良率89.66%。治疗前后比较有统计学差异(P<0.001)。术后6个月切口均Ⅰ期愈合,平均愈合时间为1.21±0.27个月,完全康复平均时间为3.72±0.31个月。6例(10.34%)下蹲时膝关节疼痛,2例(3.45%)膝关节肿胀,2例(3.45%)负重时膝关节失稳,1例(1.72%)膝关节水肿。结论:半腱肌解剖重建治疗MCL损伤临床疗效可靠,且并发症和后遗症少,值得应用推广。Objective：The aim of this study is to investigate effects of the treatment of semitendinosus an- atomic reconstruction for the medial collateral ligament （MCL） injury. Methods： 58 included /l degree of in- jury patients were performed the operation. The methods followed as knee ligament repair, superficial recon- struction and posterior oblique ligament part reconstruction. Results： Before treatment, the score of 4 cases （6.90%） were excellent, 15 cases （25. 86%）were good , 17 cases （29. 31%）were general, 22 cases （37.93%） were poor, and the good rate was 62.07% . After treatment, the score of 24 cases （41.38%）were excellent, 19 cases （32. 76%）were good , 9 cases （15.52%）were general, 6 cases （10.34%） were poor, and the good rate was 89.66%. There was a significant statistical difference between before and after treatment （P〈0.001）. 6 months after surgery, all the incisions were completely healed. The average healing time was 1.21±0.27 months , and the average time to complete recovery was 3.72±0.31 months. In an addition, 6 cases occurred knee pain when squatting, 2 cases had knee swelling, 2 cases suffered knee instability during weight-bearing and 1 case had knee edema. Conclusion： Semitendinosus anatomic reconstruction has reliable clinical efficacy treating for MCL injury, and has fewer complications and sequelae. It is worthy for promo- ting the application, application promotion .
