全身糖皮质激素联合布地奈德、异丙托溴铵雾化吸入治疗儿童闭塞性细支气管炎临床疗效分析

摘　　要：【目的】观察全身糖皮质激素联合布地奈德、异丙托溴铵雾化吸入治疗儿童闭塞性细支气管炎（bronchiolitis obliterans ，BO）的临床效果，为临床治疗提供参考依据。【方法】收集2010年1月至2014年3月来本科就诊的临床诊断为 BO 的儿童患者50例，采用随机数字表法将其随机分为观察组与对照组组各25例，两组均给予常规对症支持治疗，对照组给予全身糖皮质激素口服，观察组采用全身糖皮质激素联合布地奈德、异丙托溴铵雾化吸入治疗，测量两组患者治疗前后喘息控制测试的得分及两组患者治疗前后的临床表现总得分，比较两组患者的临床治疗效果。【结果】观察组与对照组在治疗前的喘息得分及临床表现总得分之间的差异均无统计学意义（ P ＞0．05），而治疗后观察组的喘息得分及临床表现总得分较对照组的低且差异均具有统计学意义（ P ＜0．05）；治疗后观察组的总有效率为92．0％，而对照组的总有效率为60．0％，两组之间的差异具有统计学意义（χ^2＝7．018，P ＝ 0．008）。【结论】全身糖皮质激素联合布地奈德、异丙托溴铵雾化吸入治疗儿童 BO 的临床效果显著，推荐在临床上应用。[Objective] To explore the clinical efficacy of glucocorticoids plus budesonide and ipratropium bro ‐mide for bronchiolitis obliterans （BO） so as to provide the reference for clinical treatment .[Methods] A total of 50 BO children were recruited and divided randomly into two group of observation （ n = 25） and control （ n = 25） .The observation group received glucocorticoids plus budesonide and ipratropium bromide while the control group glucocor ‐ticoids alone .The scores of breathing control test and clinical manifestations before and after treatment were compared between two groups .[Results] The wheezing scores and the total score of clinical manifestations before treatment had no inter‐group statistical significance （ P 〉 0 .05） .And the wheezing scores and the total score of clinical manifesta ‐tions were lower in observation group after treatment than control group .And the differences were statistically signifi‐cant （ P 〈 0 .05） ,The overall efficiency of observation group after treatment was 92% while the control group 60% . And statistically significant differences existed between two groups （χ^2 = 7 .018 ,P = 0 .008） .[Conclusion] The clin‐ical efficacy of glucocorticoids plus budesonide and ipratropium bromide is significant for BO .
