摘　　要：目的探讨新生儿重症监护病房(NICU)实施出院计划对早产儿的有效延续居家护理效果。方法根据家长意愿将110例早产儿分为对照组和观察组。对照组54例监护人接受传统入院宣教、护理及健康指导,观察组56例按早产儿出院计划实施健康指导和护理。比较两组父母的焦虑程度、患儿出院后的再次入院率(矫正胎龄40周)以及出院后矫正胎龄3、6、12个月的体格检查结果。结果观察组父母焦虑评分和患儿出院后再入院率低于对照组(P<0.05),早产儿矫正胎龄的3、6、12个月时身高、体质量均显著高于对照组,差异有统计学意义(P<0.05)。结论延续护理管理NICU早产儿实施出院计划,可有效降低患儿父母焦虑程度及早产儿出院后的再入院率,有利于患儿体格生长,确保患儿出院后获得持续性的照护。Objective To explore the effect of discharge planning implemented by Neonatal Intensive Care Unit（NICU） for effectively continuous of home care of preterm infants.Methods One hundred and ten preterm infants were randomly divided into control and experimental groups.The control group（n = 54） was given the routine admission education,nursing and health guidance while the experimental group（n = 56） was given the nursing and health guidance based on the discharge planning.The degree of parents＇ anxiety,the readmission rate of preterm infants（at corrected gestational age of 40 weeks）,and the data of medical examination at corrected gestational age of 3,6 and 12 months were compared between the two groups.Results Compare to the control group,the degree of parents＇ anxiety and the readmission rate of preterm infants in the experimental group were significantly decreased（P〈0.05） while infant body length and weight in the experimental group were significantly increased at corrected gestational age of 3,6,and 12 months（P〈0.05）.Conclusion The discharge planning can effectively reduce the anxiety of parents and the readmission rate of preterm infants.It can improve the growth and development of preterm infants,and also obtain a continuous care after the discharge from NICU.
