复方利多卡因乳膏缓解动脉采血疼痛的观察

摘　　要：目的观察复方利多卡因乳膏用于动脉穿刺的镇痛效果。方法选择90例拟行动脉血气分析的患者,随机分为治疗组和对照组,每组45例。治疗组于动脉穿刺操作前60 min在穿刺部位涂敷复方利多卡因乳膏,对照组采用凡士林行相同处置。穿刺后观察两组患者穿刺疼痛情况,并于5 min内完成疼痛评分。同时记录一次穿刺成功率及总穿刺成功率。记录两组局部皮肤潮红、红斑及瘙痒的发生情况。结果治疗组镇痛效果明显优于对照组,两组比较差异有统计学意义(P<0.05);治疗组疼痛评分低于对照组,两组比较差异有统计学意义(P<0.05);治疗组穿刺成功率显著高于对照组。结论动脉采血前外敷复方利多卡因乳膏能有效缓解穿刺引起的疼痛,减轻患者痛苦,提高穿刺成功率,且操作简单,不良反应少,值得推广。Objective To observe the alleviative effect of lidocaine-prilocaine cream on pain in arterial injection. Methods 90 patients were randomly divided into treatment group and control group,45 patients in each group. In treatment group,the puncture position was coated with lidocaine-prilocaine cream for 60 min before the operation,but coated with normal saline in control group. The statement of the puncture pain was observed and the visual analogue scale was recorded in 5 min after operation. The total success rate and the success rate of first puncture were analyzed simultaneously. Adverse reaction such as erubescence,erythema and itching were recorded. Results The analgesic effect of treatment group was obviously superior to that of control group,there was significant difference between the two groups（ P〈0. 05）. The visual analogue scale in treatment group was lower than that of control group（ P〈0. 05）.The success rate of puncture in treatment group was higher than that of control group. Conclusion The pain caused by arterial puncture can be alleviated by lidocaine-prilocaine cream coated before operation. It can alleviate the suffering of patients and improve the success rate of puncture. The operation is simple with less adverse reactions at the same time,so it is worthy of being generalized.
