腹腔镜手术和开腹分期手术治疗早期子宫内膜癌的效果对比

摘　　要：目的探讨腹腔镜手术和开腹分期手术治疗早期子宫内膜癌的临床效果和优缺点。方法选取2012年5月-2013年9月于本院治疗的68例早期子宫内膜癌患者作为研究对象,接受腹腔镜手术治疗的37例患者作为研究组,采用开腹分期手术治疗的31例患者作为对照组。比较两组的临床疗效和并发症发生情况。结果两组的手术时间、淋巴结切除数目差异无统计学意义（P〉0.05）;研究组的术中出血量、腹腔引流量显著少于对照组,术后排气时间、住院天数显著短于对照组,术后发热率显著低于对照组,两组差异有统计学意义（P〈0.05）,但两组的术后并发症发生率差异无统计学意义（P〉0.05）。结论腹腔镜手术和开腹分期手术治疗早期子宫内膜癌均有较好的临床效果,但腹腔镜手术具有创伤性小、患者恢复快等优点,有望完全取代传统开腹手术的方法,值得临床推广应用。Objective To explore the clinical effect,merits and faults of laparoscopic surgery and open staging surgery in the treatment of early endometrial cancer. Methods 68 patients with early endometrial cancer treated in our hospital from May 2012 to September 2013 were selected as research object.37 patients were accepted laparoscopic surgery,which was named as research group.31 patients were accepted open staging surgery,which was chosen as control group.The clinical effect and occurrence of complications in two groups was compared. Results There was no statistical difference of surgical time,number of cleaning lymph nodes during surgery（P〉0.05）.Operation blood loss and drainage of research group was significantly less than that of control group,postoperative exhaust time and hospital stay of research group was significantly shorter than that of control group,rate of fever after surgery of research group was significantly lower than that of control group,the difference was significant between two groups（P〈0.05）.There was no statistical difference of postoperative complication rate between two groups（P〉0.05）. Conclusion Laparoscopic surgery and open surgery for early endometrial cancer had better clinical results,but laparoscopic surgery had little operative wound and quick recovery,and is hopeful in replacement of traditional laparotomy,which is worthy of clinical promotion and application.
