鼻内镜下两种不同手术方式治疗儿童腺样体肥大的临床效果

摘　　要：目的比较鼻内镜下两种不同手术方式治疗儿童腺样体肥大的临床效果。方法选取本院2012年6月-2014年9月收治的74例腺样体肥大患儿作为研究对象,随机分成实验组和对照组,各37例。对照组给予鼻内镜下腺样体切除术,实验组给予鼻内镜下低温射频消融切除术,比较两组的临床效果。结果实验组的总有效率为97.30%,显著高于对照组的81.08%,差异有显著统计学意义（P〈0.05）。实验组的手术时间、鼻腔恢复至正常通气时间显著短于对照组,差异有统计学意义（P〈0.05）。实验组的出血量显著少与对照组,差异有统计学意义（P〈0.05）。实验组的并发症发生率显著低于对照组,差异有统计学意义（P〈0.05）。结论鼻内镜下低温射频消融切除术治疗儿童腺样体肥大的临床效果优于电动切割器行腺样体切除术,值得临床推广应用。Objective To compare the clinical effect of two different types of surgical procedures under nasal endoscope in the treatment of pediatric adenoidal hypertrophy. Methods 74 children patients with adenoidal hypertrophy in our hospital from June 2012 to September 2014 were selected and randomly divided into the control group and the experiment group,37 cases in each group.The control group was given adenoidectomy under nasal endoscope,the experiment group was given low-temperature radiofrequency ablation and resection under nasal endoscope.The clinical effect of two groups was compared. Results The total effective rate in the experiment group was 97.30%,which was higher than 81.08% in the control group,with significant difference（P〈0.05）.The operation time,the time of nasal cavity ventilation recovery to normal in the experimental group was shorter than that in the control group,with significant difference（P〈0.05）.The amount of bleeding in the experiment group was less than that in the control group,with significant difference（P〈0.05）.The incidence of complication in the experiment group was lower than that in the control group,with significant difference（P〈0.05）. Conclusion The clinical effect of low-temperature radiofrequency ablation and resection under nasal endoscope in the treatment of pediatric adenoidal hypertrophy is better than that of electric cutter used in adenoidectomy,it is worthy of clinical promotion and application.
