亚甲蓝排泄试验在重型颅脑损伤病人鼻饲中的应用

摘　　要：[目的]探讨亚甲蓝排泄试验在确定重型颅脑损伤病人鼻饲量及鼻饲速度中的应用效果。[方法]选取我科收治的260例重型颅脑损伤病人为研究对象,并分为试验组和对照组,试验组于受伤第3天、第8天、第15天予以亚甲蓝排泄试验,根据病人尿液变蓝的时间确定实施营养支持的方式、鼻饲量及鼻饲速度;对照组常规先采用肠内营养支持治疗,后根据胃潴留量确定鼻饲量和鼻饲速度,比较两组并发症发生情况、胃潴留发生情况、体重、血清清蛋白及血红蛋白结果及临床疗效。[结果]试验组并发症和胃潴留发生率低于对照组,体重、血清白蛋白及血红蛋白值和临床疗效高于对照组,差异有统计学意义(P<0.05)。[结论]根据亚甲蓝排泄试验来确定重型颅脑损伤病人营养支持方式、鼻饲量及鼻饲速度,对给病人提供合理的营养支持方式、减少胃潴留等并发症的发生就及改善预后有重要的临床价值。Objective：To probe into the application effect of methylene blue excretion test in determination of na-sal feeding amount and speed of severe craniocerebral injury patients.Methods：A total of 260 cases of severe craniocerebral injury patients were selected as the research objects.They were divided into test group and con-trol group.Patients in test group were given the methylene blue excretion test on the third day,the 8th day,the 15th day after injury.test then according to the blue change time of patients’urine to determine the implementa-tion method,nasal feeding amount and feeding speed of nutritional support.Patients in control group received the enteral nutrition support therapy,to determine the nasal feeding amount and speed according to the volume of gastric retention.And the occurrence of complications and gastric retention,body weight,serum albumin and hemoglobin levels and clinical efficacy were compared between both groups.Results：The incidence of complica-tions and gastric retention in test group was lower than that in control group.The body weight,serum albumin and hemoglobin values and clinical efficacy were higher than that in control group.There was statistically signif-icant difference between them （P 〈 0.05 ）.Conclusion：It has important clinical value to determine nutritional support method,nasal feeding amount and speed for severe craniocerebral injury patients on the basis of the methylene blue excretion test for providing reasonable nutritional support mode,reducing the occurrence of complications such as gastric retention and improving prognosis of patients.
