摘  要:目的:探讨糖尿病患者的泪液及其血清中高级糖基化终末产物( AGEs)和葡萄糖的浓度变化与糖尿病性视网膜病变( DR)的相关性。方法收集2013年1月至2014年1月中国康复研究中心眼科收治的39例糖尿病患者的临床资料。按照患者是否患DR,将21例患有DR的糖尿病患者作为DR组,将18例未患DR的糖尿病患者作为非DR组。采用己糖激酶法检测所有患者泪液和血清中的葡萄糖浓度,采用酶联免疫吸吸附法检测患者泪液和血清中AGEs的浓度。两组患者的空腹血糖和泪液中葡萄糖的浓度呈正态分布,以均数±标准差( x珋±s)的形式表示,并采用独立样本t检验的方法进行组间比较。泪液和血清中AGEs的浓度呈偏态分布,以中位数和上下4分位数的形式表示,并采用Ranksum秩和检验的方法进行组间比较。矫正年龄和性别两个因素,采用多元logistic回归分析,以是否患DR作为因变量,高空腹血糖、泪液高葡萄糖浓度、泪液高AGEs浓度和血清高AGEs浓度作为自变量,寻找导致DR相关的危险因素。结果 DR组患者的平均空腹血糖为(14.23±5.08) mmol/L,非DR组患者的平均空腹血糖为(10.89±3.10)mmol/L,DR组患者的空腹血糖高于非DR组,差异有统计学意义( t=2.51, P<0.05)。 DR 组患者泪液中的葡萄糖浓度为(27.48±13.09)mmol/L,非DR组患者泪液中的葡萄糖浓度为(15.00±7.36)mmol/L,DR组患者泪液中的葡萄糖浓度高于非DR组,差异有统计学意义(t=3.73,P<0.05)。 DR组患者泪液中的AGEs浓度为3.39(1.62,4.50)mmol/L,非DR组患者泪液中的AGEs浓度为2.21(0.87,3.49)mmol/L,差异无统计学意义(w=1.27,P>0.05)。 DR组患者血清中的AGEs浓度为4.81(3.45,5.53)mmol/L,非DR组患者血清中的AGEs浓度为6.08(4.63,11.75)mmol/L,差异无统计学意义�ObjectiveTo investigate the association of advanced glycation end products (AGEs) and glucose levels in tears of patients with diabetic retinopathy ( DR). Methods Twenty-one diabetes mellitus (DM) patients with DR and 18 DM controls without DR who were seeking care in the Department of Ophthalmology,China Rehabilitation Research Center in Beijing between January,2013 and January,2014 were recruited. Fasting blood and tear specimens were collected at their first visit. Glucose levels were measured by Hexokinase assays. AGEs levels were measured by enzyme-linked immunosorbent assays. Differences between groups were analyzed by t test, Ranksum test and cbi-square test using the R-software. Associations of DR with AGEs and glucose levels were assessed by logistic regression analysis. Results The average fasting blond-glucose (FBG) of DR patients group is ( 14.23±5.08 ) mmol/L, which is higher than that non-DR patients group (10. 89±3. 10) mmol/L. Owe to the, the difference have the statistically significant (t = 2.51 ,P〈0.05 ). The glucose concentration in tear of DR patients group and non-DR patients group is (27.48± 13.09 ) mmol/L and (15.00±7.36) retool/L, respectively. The glucose concentration in tear of DR patients group is higher than that non-DR patients group. The difference also have the statistically significant (t = 3.73, P 〈0.05). The AGEs concentration in tear of DR patients group is 3.39 ( 1.62, 4.50) mmol/L and the value of non-DR patients group is 2. 21 (0. 87,3.49 ) mmol/L. However, the difference do not have the statistically significant due to ( w = 1.27, P 〉0.05 ). The AGEs concentration in serum of DR patients group and non-DR patients group is 4.81 (3.45,5.53)mmoL/L and 6.08 (4.63, 11.75 )mmol/L, respectively. However, the difference do not have the statistically significant because of (w = 1.84 ,P 〉0.05 ).
