米索前列醇预防剖宫产术后出血的疗效观察

摘　　要：目的观察米索前列醇用于高龄孕妇妊娠期高血压疾病患者剖宫产预防产后出血的效果。方法选择2011年6月-2012年6月天津市职业病防治院妇产科120例择期剖宫产的高龄初产妊娠期高血压疾病产妇,无用药禁忌症,随机分为观察组和对照组,观察组60例,胎儿娩出后宫体肌内注缩宫素20U,肛内置入米索前列醇400μg;对照组60例,胎儿娩出后子宫体肌内注射20U缩宫素,同时静脉滴注20U缩宫素。观察两组产后2h及24h出血量。结果两组产后2h及24h出血量差异有统计学意义(P<0.01)。结论高龄孕妇妊娠期高血压疾病患者择期剖宫产术应用米索前列醇400μg置肛内促进子宫收缩强于缩宫素,能有效预防产后出血,且用药方便,副反应少。Objective To observe the effect of Misoprostol Used in the prevention of postpartum hemorrhage in the elderly pregnant women with hypertension during pregnancy. Methods June 2011 to 2012 June Tian- jin city occupation disease prevention and treatment hospital department of gynaecology and obstetrics, 120 cases of elective cesarean section of elderly primipara with pregnancy hypertension and maternal, useless medicine contraindication, were randomly divided into observation group and control group, 60 cases in ob- servation group, fetal childbirth uterine muscle in injection of oxytocin 20U, anal placement misoprosto1400 μg; 60 cases in the control group, after the delivery of the fetus within the uterine muscle injection 20U oxy- tocin, also intravenous injection 20U oxytocin. The amount of 2h and 24h bleeding in two groups were ob- served. Results The differences of 2h and 24h bleeding volume of the two groups were statistically significant （P〈0.01）. Conclusions Older pregnant women, pregnancy hypertension disease patients undergoing elec- tive cesarean section to produce the technique application of misoprostol 400μg set inside the anus to pro- mote uterine contraction than that of oxytocin to prevent postpartum hemorrhage, and convenient in use, and fewer side effects.
