检测血清降钙素原和C反应蛋白在诊断感染性疾病中的价值

摘　　要：目的评价血清降钙素原(PCT)和C反应蛋白(CRP)在诊断感染性疾病中的价值。方法收集2014年4—9月本院收治的感染性疾病患者106例,根据病史、临床表现、体征和辅助检查作出诊断,分为重症感染组,局部感染组和病毒感染组,用免疫荧光法测血清PCT含量,用免疫比浊法测血清CRP含量。结果与病毒感染组相比,重症感染组和局部感染组的血清PCT和CRP水平均存在显著性差异(P<0.05);与局部感染组相比,重症感染组的血清PCT水平存在显著性差异(P<0.05),而血清CRP水平差异不明显(P>0.05)。以PCT>0.5 ng/ml为阳性标准,诊断细菌感染的敏感性和特异性分别为90.4%和93.9%,准确性为91.5%;以CRP>8 mg/L为阳性标准,诊断细菌感染的敏感性和特异性分别为87.6%和87.9%,准确性为87.7%。结论 PCT和CRP测定对感染性疾病有一定的辅助诊断价值,与CRP相比,PCT具有更高的敏感性、特异性和准确性。Objective To evaluate the value of serum procalcitonin （ PCT ） and c-reactive protein （CRP） in diagnosing infectious diseases .Methods During April 2014 and September 2014, 106 patients with infectious diseases who treated in our hospital were enrolled in this study , the diagnosis was made according to the disease history , clinical symptoms , signs and auxiliary examination , and they were divided into severe infection group, local infection group and viral infection group .Immunofluorescence method was used to measure serum PCT levels, immune turbidimetric method was used to measure the serum CRP levels .Results The differences of serum PCT and CRP level between viral infection group and severe infection group and the local infection group were statistically significant （P〈0.05）.The difference of serum PCT level between local infection group and severe infection group was statistically significant （P〈0.05）.In diagnosing bacterial infection, the sensitivity and specificity of PCT （〉0.5ng/ml） were 90.4%and 93.9%respectively, the accuracy was 91.5%.And the sensitivity and specificity and accuracy of CRP （ 〉8mg/l） in diagnosing bacterial infection were 87.6% and 87.9%and 87.7% respectively.Conclusions The determination of PCT and CRP has auxiliary diagnosis for infectious diseases .And PCT has higher sensitivity , specificity and accuracy .
