摘 要:目的探讨血清CTRP-9水平和内脏脂肪分布部位对代谢综合征(MS)的影响。方法选取88例代谢综合征患者,分为MS组42例,对照组46例。通过64排双源心脏CT测量心外膜脂肪体积(EATV)。ELISA法检测血清CTRP-9、脂联素(APN)及瘦素水平。结果 MS组患者的腰围、BMI、EATV、TG、空腹血糖、HOMA-IR明显高于对照组,而HDL-C、APN和CTRP-9水平显著低于对照组(P<0.05);EATV、腰围和BMI对MS均有诊断预测价值;调整血清APN水平后,血清CTRP-9与空腹血糖、EATV仍显著负相关(r=-0.390,P<0.001;r=-0.273,P=0.033),与HDL-C显著正相关(r=0.208,P=0.041);EATV是影响MS的独立危险因素,而HDL-C、脂联素和CTRP-9是MS的保护因素。结论血清CTRP-9与EATV和代谢参数存在良好的相关性,此相关性独立于血清APN水平。EATV是影响MS的独立危险因素,而CTRP-9是保护因素。Objective To investigate the effects of serum CTRP-9 levels and visceral fat distribution on the metabolic syndrome( MS). Method A total of 88 patients were collected from February 2013 to October 2013. Patients were divided into the metabolic syndrome group( 42 cases) and control group( 46 cases). Epicardial adipose tissue volumes( EATV) were measured by dual-source 64-slice CT. Fasting blood sample were collected for biochemical examination and cytokines by ELISA. Result The waist circum-ference,body mass index,EATV,TG,fasting glucose and HOMA-IR in patients with MS were significantly higher than those in patients with non-metabolic syndrome,HDL-C,adiponectin and CTRP-9 in patients with MS were significantly lower than those in patients with non-metabolic syndrome( P〈 0. 05); The area under receiver operating characteristic( ROC) curve of EATV,waist circumference and BMI in diagnosis of MS were 0. 668( P = 0. 007),0. 797( P〈 0. 001) and 0. 652( P = 0. 014); After adjust the serum adiponectin level,serum CTRP-9 and fasting glucose,EATV were significantly negative correlation( r=- 0. 390,P〈 0. 001 and r =- 0. 273,P = 0. 033),and HDL- C was significantly positive correlation( r = 0. 208,P = 0. 041); EATV was an independent risk factor for MS,HDL-C,adiponectin and CTRP-9 were protective factors for MS. Conclusion There were good correlations between serum CTRP-9 and EATV,and other metabolic parameters,the correlations were independent of the serum adiponectin levels. EATV was an independent risk factor and CTRP-9 was a protective factor for MS.
