摘 要:目的了解综合性ICU重点部位医院感染特点。方法采用综合ICU目标性监测方法,统计2011年6月—2012年12月患者医院感染发生情况,重点监测侵入性操作感染发生率。结果监测患者2 591例,共发生210例次医院感染,例次感染率为8.10%,例次日感染率为32.52‰,调整日感染率为9.95‰;呼吸机相关性肺炎(VAP)日感染率37.73‰,留置导尿管相关泌尿道日感染率(CAUTI)5.47‰,动静脉插管相关性血流日感染率(CRBSI)2.81‰;共检出214株病原菌,以革兰阴性菌为主,占71.96%。结论 ICU医院感染发病率高,主要原因是侵入性操作较多;目标性监测能准确全面了解ICU医院感染动态变化,及时制定有效防控措施,降低医院感染发生率。Objective To explore the nosocomial infection occurrence related to comprehensive ICU apparatus.Methods By applying targeted surveillance in comprehensive ICU,patient’s infection occurrence from Jun 2011 to Dec 2012 was investigated and statistical analysis was performed.Results Among 2 591 inpatients in ICU,210 suffered from nosocomial infection.The infection rate was 8. 10%,the infection rate per day was 32.52‰,and the daily infection rate was 9.95‰ after the adjustment with ASIS method.The daily infection rates were as follows:37.73% in ventilator associated pneumonia (VAP),5.47‰ in catheter associated urinary tract infec-tion (CAUTI),2.81‰ in catheter related bloodstream infection(CRBSI).Among 214 detected strains,the Gram negative bacteria ac-counted for 71.96%.Conclusions Nosocomial infection rate is high in ICU,of which 3 kinds of invasive procedure associated infec-tions are the major ones.Dynamic information of nosocomial infection can be acquired accurately in time through target monitoring,and effective intervention measures can be set down to reduce the incidence of nosocomial infection.
