多囊卵巢综合征患者夫精人工授精前预处理的价值研究

摘　　要：目的:对多囊卵巢综合征患者夫精人工授精前预处理的价值进行研究。方法:选取2012年1月-2013年12月在本院行夫精人工授精的多囊卵巢综合征患者100例,分为两组:观察组50例夫精人工授精前给予炔雌醇环丙孕酮片联合二甲双胍肠溶片预处理治疗,对照组50例直接行夫精人工授精,比较两组患者的内分泌水平及妊娠结局。结果:观察组患者治疗后LH、LH/FSH、T和SHBG水平均降低,明显低于治疗前和对照组;观察组内膜厚度(HCG日)、排卵周期数及临床妊娠率均明显高于对照组,卵泡数明显少于对照组;观察组流产率及并发症发生率均低于对照组。以上各项比较差异均有统计学意义(P<0.05)。两组E2(HCG日)和HMG总用量比较,差异无统计学意义(P>0.05)。结论:对PCOS患者夫精人工授精前给予炔雌醇环丙孕酮片联合二甲双胍预处理,可以改善患者的内分泌和代谢异常状态,提高卵子质量和临床妊娠率,降低流产率和妊娠期并发症的发生。Objective：To study the value of pretreatment before artificial insemination by husband（AIH） in polycystic ovary syndrome patients.Method：100 patients with polycystic ovary syndrome in our hospital from January 2012 to December 2013 were selected and divided into two groups.50 cases in the observation group were given pretreatment of ethinylestradiol and cyproterone acetate tablets and metformin before AIH,50 cases in the control group were directly given AIH.The endocrine level and pregnancy outcome were compared between the two groups.Result：After treatment,the levels of LH,LH/FSH,T and SHBG in the observation group decreased,which were significantly lower than those before treatment and those in the control group,the differences were statistically significant（P〈0.05）.Compared with the control group,the intimal thickness（HCG day）,rate of pregnancy and number of ovulatory cycle in the observation group were significantly higher,the number of follicles of the observation group was significantly lower,the differences were statistically significant（P〈0.05）.The rate of abortion and the occurrence of complications in the observation group were significantly lower than those of the control group（P〈0.05）.About the level of E2（HCG day） and the total amount of HMG,there was no difference between the two groups（P〉0.05）.Conclusion：Pretreatment with ethinylestradiol and cyproterone Acetate Tablets and metformin before AIH in polycystic ovary syndrome patients can not only improve the abnormal state of endocrine and metabolism,the quality of ovum and the rate of clinical pregnancy but also decrease the rate of abortion and the occurrence of complications.
