摘　　要：目的探讨单侧椎弓根钉固定并椎间融合治疗腰椎间盘突出症的临床疗效。方法回顾性分析2010年1月至2013年1月采用单侧椎弓根钉固定椎间融合治疗43例腰椎间盘突出症患者,其中男26例,女17例;年龄40-56岁,平均48.2岁。记录手术时间、失血量、住院时间,分别于术后2周、3个月、1年通过X线片、VAS评分、JOA评分评估临床疗效。结果 43例均获随访,时间12-30个月,平均15.2个月。VAS评分：术前（8.4±1.3）分,术后2周、术后3个月、术后1年分别为（1.21±0.41）分、（1.11±0.32）分、（1.09±0.29）分,与术前比较差异均有统计学意义（P〈0.05）。JOA评分：术前（16.19±2.70）分,术后2周、3个月、1年分别为（22.74±2.19）分、（24.30±1.49）分、（27.74±1.22）分,与术前比较差异均有统计学意义（P〈0.05）。所有病例术后症状均明显缓解。结论单侧椎弓根钉固定并椎间融合是治疗腰椎间盘突出症的良好方法,缩短了手术时间、减少了创伤,手术疗效确切。Objective To evaluate the effect of unilateral pedicle screw fixation and intervertebral fusion for lumbar disc herniation（ LDH）. Methods From January 2010 to January 2013,43 cases of patients with LDH were treated by unilateral pedicle screw fixation and intervertebral fusion,there were 26 males and 17 females,with age of 40 to 56 years old（ average 48.2 years old）. The operation time,blood loss,hospital stay were recorded. At 2 weeks,3 months,1 year after operation,X-ray film,VAS score,JOA score were used to evaluate the clinical effect. Results 43 cases were followed up for 12 - 30 months,with an average of 15. 2 months. VAS score were（ 8. 4 ± 1. 3） points preoperative,and（ 1. 21 ± 0. 41）,（ 1. 11 ± 0. 32） points,（ 1. 09 ± 0. 29） points at 2 weeks,3 months and 1 year after operation respectively. The difference were statistically significant comparedwith VAS score before operation（ P〈0. 05）. JOA score was（ 1 6. 1 9 ± 2. 7 0） points preoperative and（ 2 2. 7 4 ±2. 19） points,（ 24. 30 ± 1. 49） points,（ 27. 74 ± 1. 22） points 2 weeks,3 months and 1 year after operation respectively. The difference were statistically significant compared with JOA score before operation（ P〈0. 05）. The symptoms were significantly improved. Conclusion Treatment of LDH by unilateral pedicle screw fixation and intervertebral fusion is a good method for treating LDH with definite effect,which shortening the operation time and reducing trauma.
