北京市某三甲医院慢性阻塞性肺疾病住院患者直接经济负担影响因素分析

摘　　要：目的:分析某院慢性阻塞性肺疾病住院患者相关数据,探讨慢阻肺住院患者直接经济负担的影响因素。方法:选取2005年1月1日-2013年12月31日在北京市某医院住院的1638例慢阻肺确诊患者作为研究对象。以住院患者产生的可比价格医疗费用表示直接经济负担,对其进行对数转换后转化为正态分布资料,采用多元线性逐步回归法筛选慢阻肺患者直接经济负担的影响因素。结果:2005-2013年慢性阻塞性肺疾病住院患者人均直接经济负担从19766.52元增长到26133.18元,平均增长速度为3.55%;调整价格因素后,可比价格人均直接经济负担年均增长0.39%。多元回归结果显示,年龄、转科、药物过敏、住院次数、住院天数、入院病情、抢救、出院转归情况、病人来源是直接经济负担的主要影响因素(F=146.722,P<0.01,R2=0.607)。从偏回归系数可知,住院天数对患者直接经济负担的影响最大。结论:慢阻肺住院患者的直接经济负担主要来自患者自身的疾病严重程度,降低直接经济负担应从控制慢阻肺患者的病情入手。Objective： To analyze the data of hospitalized patients with COPD in order to discuss the influencing factors of these patients＇ direct economic burden. Methods： 163g hospitalized patients diagnosed with COPD were selected from January 1,2005 to December 3l, 2013 as the research object. The hospitalized patients＇medlcal treatment costs represent the direct economic burden, which were converted to normal distribution data with logarithmic transformation. Multiple linear stepwise regression method is used to filter the influence factors of direct economic burden of COPD patients. Results： The per capita direct economic burden of COPD hospitalized patients increased from 19766.52 RMB to 26133.18 RMB during 2005 -2013, accompanying the average growth of 3.55%. The average growth rate is 0.39% after adjusting the price factors. The outcome of multiple linear stepwise regression reveals that age, rescue, hospital transference, drug allergy, durations of hospitalization, times of hospitalization, patient＇s condition, discharge outcome and source of patients are the main influence factors of the direct economic burden （ F = 146. 722, P 〈 0. 01, R^2 =0.607）. Conclusion： The direct economic burden of hospitalized patients with COPD mainly depends on the severity of patients＇disease.
