2008-2012年重庆市九龙坡区艾滋病自愿咨询检测分析

摘　　要：目的了解并掌握重庆市九龙坡区艾滋病自愿咨询检测（Voluntary Counseling and Testing,VCT）人群的基本情况和感染状况,为今后艾滋病防治干预工作的重点和方向提供科学依据。方法收集重庆市九龙坡区2008-2012年艾滋病自愿咨询检测点上报的《检测咨询个案登记表》,网络下载后建立Microsoft Excel数据库,采用SAS8.2软件对数据进行Chi-Square分析。结果 2008-2012年重庆市九龙坡区各VCT点求询者共计44 821人,男女之比为0.88∶1,女性多于男性,年龄以20~40岁为主,婚姻状况以未婚为主,学历以初中为主;求询者最多的是有非婚异性性行为史者,为29 244人,占总的求询者的65.25%,其次为有注射吸毒史者占14.59%;HIV阳性数为438人,总的检出阳性率为0.98%,各年间差异有统计学意义,检出阳性率最高的是配偶/固定性伴阳性者,达11.64%,各年间比较差异无统计学意义,其次为有男男性行为史者,阳性检出率为9.53%,各年间差异有统计学意义,有注射吸毒史者阳性检出率为1.71%,各年间差异有统计学意义;从一般情况与检出阳性率看,30~40岁阳性检出率最高,为1.36%,其次为60岁以上人群检出率1.13%;从婚姻状况来看,离异或丧偶的检出率最高,为1.50%;从学历来看,大专以上学历检出率最高,为1.75%。结论以非婚性行为后前来求询的咨询者占有相当大的比例,包括非婚异性性接触和男男性行为,占67.92%。配偶/固定性伴阳性者、男男性行为人群等人群HIV阳性检出率较高,下一步将进一步加大该人群的检测和干预,控制艾滋病经性途径的传播。Objective To understand the infection status of voluntary counseling and testing（ VCT） clients in Jiulongpo district of Chongqing,and to provide a scientific basis for further pertinent intervention measures.Methods The VCT case reports in 2008 to 2012 were collected from the VCT sites and were statistically analyzed with Microsoft Excel and SAS8. 2. Results In Total,there were 44 821 clients received the VCT service in Jiulongpo district,Chongqing from 2008 to 2012. The gender ratio of male to female was 0. 88： 1. Most of the clients were aged from 20 to 40 years and mainly were unmarried. Most of their educational level was junior middle school. Among the clients, most of them had non-marital heterosexual behavior history, accounted for 65. 25%（ 29 244） of all clients. The clients with drug injection history accounted for 14. 59%. There were 438 clients tested as HIV-positive,with a positive rate of 0. 98%. The difference among years showed statistical significance. The clients with HIV-positive spouse had the most high infection rate as 11. 64%,with no significant statistical difference among years. The positive rate of men who have sex with men（ MSM） was the second（ 9. 53%）. The infection rate among drug users was 1. 71%. In terms of age,marriage status and education background,the infection rate of those with age of 30 to 40 years,divorced or widowed,and with college educational level or above were the highest as 1. 36%,1. 50% and 1. 75%,respectively. Conclusion The clients after non-marital sexual behaviors take a considerable proportion the all the clients,including non-marital heterosexual behaviors and MSM,accounting for 67. 92% of all the clients. The HIV positive rate of clients with HIV-positive spouse and MSM are relatively high. Thus,pertinent countermeasures should be implemented in those vulnerable populations to control the transmission routes through sex.
