摘　　要：目的探讨2型糖尿病患者血清胱抑素C(Cys C)与视网膜病变之间的关系,并分析其在糖尿病视网膜病变诊断中的临床价值。方法选取360例2型糖尿病住院患者和149例健康对照者(NC组),2型糖尿病患者中不伴视网膜病变(NDR组)190例、伴视网膜病变(DR组)170例,检测各组腰臀比、空腹血糖(FBG)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、糖化血红蛋白(HbA1c)、Cys C等指标,分析Cys C与2型糖尿病视网膜病变的关系。结果 2型糖尿病患者组FBG、TC、TG、LDL-C、HbA1c、Cys C高于NC组(P<0.05);DR组血清Cys C水平明显高于NDR组(P<0.05);同时多元logistic回归结果表明,2型糖尿病视网膜病变的发生发展除了与年龄和糖尿病病程等呈正相关外,还与血清Cys C水平呈正相关(β=0.791,P<0.05)。结论高血清胱抑素C水平是2型糖尿病视网膜病变的独立危险因素。Objective To investigate the relationship between serum cystatin C（Cys C）and diabetic retinopathy in type 2diabetic patients,and analyze its clinical value in the diagnosis of diabetic retinopathy.Methods 360 diabetic inpatients（190none DR,170DR）and 149 healthy people as normal control（NC）were selected.Waist-to-hip ratio,fasting blood glucose（FBG）,total cholesterol（TC）,triglycerides（TG）,high density lipoprotein cholesterol（HDL-C）,lower density lipoprotein cholesterol（LDL-C）,glycosylated hemoglobin A1c（HbA1c）,Cys C and other related indicators of the subjects were determined,and relationship between serum Cys C and type 2diabetic retinopathy was analyzed.Results The levels of FBG,TC,TG,LDL-C,HbA1 c and Cys C of type 2diabetes group were obviously higher than those of NC group（P〈0.05）.The serum Cys C of DR group was obviously higher than that of NDR group（P〈0.05）.Multivariate logistic regression showed that the occurrence of type 2diabetic retinopathy was also related to serum Cys C level（β=0.791,P〈0.05）,in addition to age and diabetes duration.Conclusion High serum Cys C level was an independent risk factor of retinopathy in type 2diabetic population.
