新诊断2型糖尿病伴高尿酸血症的相关因素分析

摘　　要：目的探讨新诊断2型糖尿病(T2DM)患者伴高尿酸血症(HUA)的相关因素。方法选取新诊断T2DM患者158例,根据血尿酸(SUA)水平(男性SUA≥420μmol/L,女性SUA≥360μmol/L),将其分为正常尿酸(NUA)组126例,HUA组32例。比较两组临床指标的差异。结果新诊断2型糖尿病患者HUA患病率为20.3%,其中男性26.3%,女性11.1%;HUA组患者舒张压(DBP)、体质量指数(BMI)、腰围(WC)、甘油三酯(TG)、血肌酐(SCr)、血尿素氮(BUN)、24小时尿微量白蛋白(UAER)均高于NUA组(P<0.05),而高密度脂蛋白胆固醇(HDLC)低于NUA组(P<0.05),两组患者糖尿病病程、空腹血糖(FPG)、餐后2小时血糖(2hPG)、糖化血红蛋白(HbA1c)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、收缩压(SBP)差异无统计学意义(P>0.05)。直线相关分析显示,TG、HDL-C、BMI、WC、DBP、SCr及UAER是高尿酸血症的影响因素(P<0.01)。结论新诊断T2DM患者高尿酸血症与高血压、血脂异常、肥胖和肾脏病变相关。Objective To investigate the factors relating to hyperuricemia in patients with new-onset type 2diabetes mellitus（T2DM）.Methods A total of 158 newly diagnosed T2 DM patients were selected and divided into normal serum uric acid（NUA）group（n=126）and hyperuricemia（HUA）group（n=32）in terms of serum uric acid levels.The clinical indices of two groups were compared.Results The prevalence of HUA in patients with new-onset T2 DM was 20.3%,including 26.3%in males and 11.1%in females.Diastolic blood pressure（DBP）,body mass index（BMI）,waist circumference（WC）,triglycerides（TG）,serum creatinies（SCr）,blood urea nitrogen（BUN）,24 hour urinary albumin excretion rate（UAER）of HUA group were significantly higher than those of NUA group（P〈0.05）,while,high density lipoprotein cholesterol（HDL-C）was obviously lower than that of NUA group（P〈0.05）.There were no statistical significance in diabetes course,fasting plasma glucose（FPG）,2hour postprandial plasma glucose（2hPG）,hemoglobin A1c（HbA1c）,total cholesterol（TC）,low density lipoprotein cholesterol（LDL-C）and systolic blood pressure（SBP）between two groups（P〉 0.05）.According to multiple linear regression,TG,HDL-C,BMI,WC,DBP,SCr and UAER were influence factors of hyperuricemia（P〈0.01）.Conclusion Hyperuricemia is highly correlated with hypertension,dyslipidemia,obesity and renal lesions in patients with new-onset T2 DM.
