结核感染T细胞酶联免疫斑点试验结果在肺结核治疗中的变化

摘　　要：目的观察结核感染T细胞酶联免疫斑点试验（T—SPOT．TB）结果在肺结核治疗中的变化。方法选取在复旦大学附属华东医院及上海市肺科医院就诊的42例初治肺结核患者，分别检测其入院时的T-SPOT．TB斑点值，以及经过标准抗结核治疗1个月和3个月后的斑点值，观察其变化。治疗3个月后，同时作痰培养。结果经标准抗结核治疗1个月和3个月后，患者的T-SPOT．TB平均斑点值均较治疗前明显下降（P〈0．05）。在治疗3个月后，9例患者痰培养仍为阳性，痰培养持续阳性患者的平均斑点值显著高于转阴患者（P〈0．05）。结论T-SPOT．TB的斑点值可在肺结核标准抗结核治疗的进程中逐渐下降，且对评价患者的预后具有一定价值。Objective To study the change of T-SPOT. TB results during the treatment of pulmonary tuberculosis. Methods Forty-two cases of pulmonary tuberculosis without treatment in Huadong Hospital affiliated to Fudan University and Shanghai Pulmonary Hospital were enrolled in the research. T-SPOT. TB assay was detected in the patients before treatment, and after standard antituberculosis therapy for one month and three months,respectively. After treatment for three months, sputum culture was also performed. Results After standard antituberculosis therapy for one month and three months,average number of spots in T-SPOT. TB assay both decreased considerably compared with that before treatment （ P 〈0.05）. After treatment for three months, the result of sputum culture was positive in nine patients,in whom Mycobaeterium tuberculosis-specific T cells were more than those in the negative ones （ P 〈0.05）. Conclusions The number of spots in T-SPOT. TB assay decreases during the standard antitubereulosis therapy of pulmonary tuberculosis, which is beneficial in evaluating the prognosis of patients.
