C-反应蛋白及前白蛋白与降钙素原检测在患儿感染诊断中的价值研究

摘　　要：目的研究降钙素原(PCT)、C-反应蛋白(CRP)和前白蛋白(PA)在儿童感染性疾病中的诊断价值,提高对感染诊断的特异性和灵敏性。方法分析2011年10月-2013年10月在医院接受住院治疗的80例感染患儿临床资料,根据血清学检查结果将患儿分为细菌组44例和病毒组36例,并选取正常体检儿童50名作为对照组,比较3组受试儿童的CRP、PA和PCT水平。结果感染组患儿的CRP(t=12.39,P<0.01)、PCT(t=16.17,P<0.01)均显著高于对照组,而PA(t=14.66,P<0.01)显著低于对照组;细菌组患儿的CRP(t=11.05,P<0.01)、PCT(t=24.75,P<0.01)均显著高于病毒组,而PA(t=18.29,P<0.01)显著低于病毒组。结论 CRP、PA、PCT联合检测能够显著提高对感染尤其是细菌性感染的诊断特异性和灵敏性。OBJECTIVE To study the value of joint detection of C-reactive protein（CRP）,prealbumin（PA）,and procalcitonin（PCT）in diagnosis of children with infectious diseases so as to improve the specificity and sensitivity of diagnosis of infections.METHODS The clinical data of 130 children with bacterial infections who were hospitalized from Oct 2011 to Oct 2013 were analyzed,the children were divided into the bacteria group with 44 cases and the virus group with 36 cases according to the results of serological examination,and 50 healthy children who received physical examination were recruited as the control group.The levels of CRP,PA,and PCT were observed and compared among the three groups of participants.RESULTS The levels of CRP（t=12.39,P〈0.01）and PCT（t=16.17,P〈0.01）of the infection group were significantly higher than those of the control group,however,the level of PA（t=14.66,P〈0.01）of the infection group was significantly lower than that of the control group.The levels of CRP（t=11.05,P〈0.01）and PCT（t=24.75,P〈0.01）of the bacteria group were significantly higher than those of the virus group,however,the level of PA（t=18.29,P〈0.01）of the bacteria group was significantly lower than that of the virus group.CONCLUSIONThe joint detection of CRP,PA,and PCT can significantly improve the specificity and sensitivity in diagnosis of the infections,especially the bacterial infections.
