摘 要:目的探讨细菌性角膜炎的病原菌分布及耐药性,为临床合理用药提供依据。方法对2011年5月-2013年5月87份细菌性角膜炎患者送检标本进行细菌培养,并行药敏试验,参照NCCLS标准判定药敏结果,采用SPSS19.0软件进行数据统计分析。结果 87份标本经细菌培养阳性28份,阳性率为32.18%;检出革兰阳性球菌15株占53.58%其中表皮葡萄球菌占21.43%、金黄色葡萄球菌占10.71%、肺炎链球菌占7.14%;革兰阴性杆菌9株占32.14%,其中大肠埃希菌占10.71%,铜绿假单胞菌占7.14%;革兰阳性球菌对青霉素G、亚胺培南、氨苄西林的耐药性最高,分别为90.91%、54.55%、54.55%,对万古霉素和利奈唑胺的耐药性最低,均为0;革兰阴性杆菌对氨苄西林、头孢呋辛、头孢曲松、氨曲南的耐药性最高,分别达85.71%、85.71%、57.14%、42.86%;对头孢他啶耐药性最低,为0。结论葡萄球菌属和大肠埃希菌是细菌性角膜炎较常见的致病菌,两种细菌的耐药性广泛,临床用药时应正确选择。OBJECTIVE To explore the distribution and drug resistance of pathogens causing bacterial keratitis so as to provide guidance for reasonable clinical use of antibiotics.METHODS The bacterial culture was performed for the 87 submitted specimens that were obtained from the patients with bacterial keratitis who were treated from May 2011 to May 2013,then the drug susceptibility testing was carried out,the results of the drug susceptibility testing were interpreted by referring to NCCLS criteria,and the statistical analysis of data was performed with the use of SPSS19.0software.RESULTS Of 87 specimens,28were positive for bacterial culture,with the positive rate of 32.18%.Totally 15(53.58%)strains of gram-positive cocci have been isolated,of which 21.43% were the Staphylococcus epidermidis,10.71% were the Staphylococcus aureus,and 7.14% were the Streptococcus pneumoniae.Totally 9(32.14%)strains of gram-negative bacilli have been isolated,of which 10.71% were the Escherichia coli,and 7.14% were the Pseudomonas aeruginosa.The drug resistance rates of the gram-positive cocci to penicillin G,imipenem,and ampicillin were 90.91%,54.55%,and 54.55%,respectively;the drug resistance rates to vancomycin and linezolid were 0.The drug resistance rates of the gram-negative bacilli to ampicillin,cefuroxime,ceftriaxone,and aztreonam were 85.71%,85.71%,57.14%,and 42.86%,respectively;the drug resistance rate to ceftazidime was 0.CONCLUSION The Staphylococcus sppand E.coli are the common pathogens causing bacterial keratitis and are highly drug-resistant,and it is necessary for the hospital to reasonably use antibiotics.
