急诊ICU感染性休克脉压与预后的相关性研究

摘　　要：目的研究急诊ICU内难治性感染休克患者脉压水平与患者预后的关系,提高对感染休克疾病的认识与治疗水平。方法对2011年4月-2013年6月急诊ICU收治的80例难治性感染性休克患者,根据4周内患者的死亡情况将80例患者,分为存活组56例、死亡组24例;测定两组患者的平均动脉压、APACHEⅢ评分的差异。结果存活组与死亡组患者的平均动脉压分别为(74.83±6.58)与(62.26±6.01)mmHg,存活组明显高于对照组,APACHEⅢ评分存活组与死亡组患者分别为(54.36±6.12)、(98.47±9.06)分,死亡组明显高于存活组,两组比较差异有统计学意义(P<0.05);存活组患者平均动脉压随着时间的延长有逐渐升高的趋势,死亡组患者平均动脉压则出现逐渐下降的趋势;死亡组患者平均动脉压与APACHEⅢ评分之间呈负相关。结论通过脉压变化能及时反应患者的预后,并且准确性高、简单方便。OBJECTIVE To study the relationship between the arterial pressure of the patients with refractory septic shock of emergency ICU and the prognosis so as to raise the awareness of septic shock and improve the treatment.METHODS A total of 80 patients with refractory septic shock who were treated in the emergency ICU from Apr2011 to Jun 2013 were enrolled in the study and divided into the survival group with 56 cases and the death group with 24 cases according to the status of death within 4weeks.The mean arterial pressure of the two groups of patients was determined,and the difference in the APACHEⅢ score was observed.RESULTS The mean arterial pressure of the survival group was（74.83±6.58）mmHg,significantly higher than（62.26±6.01）mmHg of the death group.The APACHEⅢ score of the death group was（98.47±9.06）points,significantly higher than（54.36±6.12）points of the survival group（P〈0.05）.The mean arterial pressure of the survival group showed an upward trend with the time,while the mean arterial blood pressure of the death group showed a downward trend;the mean arterial pressure of the death group was negatively correlated with the APACHEⅢ score.CONCLUSIONThe change of the arterial pressure can predict the prognosis of the patients,and it is accurate,simple,and convenient.
