卵巢功能减退者抗卵巢抗体和抗心磷脂抗体水平以及对恢复卵巢功能提示作用

摘　　要：目的 探讨卵巢功能减退者抗卵巢抗体（AOAb）、抗心磷脂抗体（ACA）水平检测的意义及对恢复卵巢功能提示作用。方法 对卵巢功能减退患者的AOAb、ACA进行测定,同时将AOAb与ACA测定结果显示阳性的患者,予以免疫抑制剂和阿司匹林治疗的预处理,同时检测排卵情况和卵泡刺激素（FSH）、促黄体生成素（LH）和雌二醇（E2）水平。结果 两组AOAb和ACA阳性率比较：两组AOAb和ACA总阳性率比较,差异有统计学意义（P〈0.05）;治疗组患者卵巢有优势卵泡出现,其中中有3例AOAb抗体阳性者在地塞米松停止治疗后第2~3周来月经;1例AOAb阳性患者自然行经2次后妊娠;2例AOAb阳性患者恢复月经规律,月经第3天FSH降至正常范围。治疗组治疗前后E2比较,差异无统计学意义（P〉0.05）,FSH、LH比较,差异有统计学意义（P〈0.05）。结论 对AOAb与ACA检测呈现阳性的患者采用免疫抑制剂和阿司匹林干预治疗对卵巢功能的恢复具有积极意义。Objective To study the level of ovary decreased ovarian antibody（ AOAb） resistance,anti cardiolipin antibody（ ACA） detection of meaning and function to restore ovarian function prompts,and provide clinical reference for treatment of ovarian function decline. Methods The patients with ovarian function declining,the AOAb and ACA was measured. At the same time,AOAb with ACA determination results show positive patients,the pretreatment of the immunosuppressive therapy,at the same time of ovulation monitoring. Results After the comparison of AOAb and ACA of the two groups,the AOAb and total positive rate of ACA is similar between the two groups,with statistically significant（ P〈0. 05）. 3 cases of treatment group,the AOAb antibody positive stop in dexamethasone treatment of 2 ~ 3 weeks after menstruation. 2 cases AOAb positive patients restore menstrual rule. The menstruation FSH dropped to the normal range at the third day. The E2 of the treatment group before and after treatment showed no statistical differences（ P〉0. 05）. Comparing the FSH,LH,the difference was statistically significant（ P〈0. 05）. Conclusion The AOAb patients with positive ACA detection by immune inhibitor intervention on the recovery of ovarian function has positive significance.
