等渗和高渗白蛋白治疗低蛋白血症的药物经济学研究

摘　　要：目的通过对等渗白蛋白(5%人血白蛋白)与高渗白蛋白(20%人血白蛋白)治疗低蛋白血症进行经济学比较,为临床合理使用白蛋白提供参考。方法收集嘉兴市第一医院确诊为低蛋白血症患者60例,其中等渗白蛋白(5%人血白蛋白)与高渗白蛋白(20%人血白蛋白)各30例,运用药物经济学中的成本-效果分析等方法对其进行分析。结果 5%人血白蛋白和20%人血白蛋白治疗低蛋白血症的成本分别为16 314.2元和15 362.1元,临床有效率分别为63%和70%(P>0.05),其成本-效果比分别为257.7和219.5;进行敏感度分析,其成本-效果比分别为254.5和216.5元;20%人血白蛋白治疗低蛋白血症期间出现1起不良反应,5%人血白蛋白未有发生。结论高渗白蛋白(20%人血白蛋白)治疗低蛋白血症的成本-效果优于等渗白蛋白(5%人血白蛋白)。Objective To provide reference for clinical rational albumin use, the isotonic albunin （5% human albumin） and hypertonic albnmin（20% human albumin） in the treatment of hypoproteinemia were conducted by economic analy- sis. Methods The clinical data of the patients administrated with 5% human albumin and 20% human albumin were collected in our hospital, and cost-effectiveness analysis and other methods were applied for the cases. Results Cost of 5% human albumin and 20% human albumin in treatment of hypoproteinemia was 16 314. 2 and 15 362. 1 yuan,clinical efficiency was 63% and 70% （P 〉 0. 05） respectively, and the cost-effectiveness rate was 257.7 and 219. 5. The cost-ef- fectiveness rate was 254. 5 and 216. 5 yuan, respectively. One case of adverse reaction had happened in the treatment of hypoproteinemia by 20% human albumin. Conclusion The cost-effectiveness of hypertonic albumin（20% human albumin） was better than that of isotonic albunin（5% human albumin） in the treatment of hypoproteinemia.
