集束化护理干预在急性重症胰腺炎肠内营养中的应用

摘　　要：目的探讨集束化护理干预在急性重症胰腺炎早期经鼻肠管肠内营养中的应用效果。方法将190例急诊重症监护室(EICU)急性重症胰腺炎患者就诊时随机地(随机表)分为对照组和观察组各95例患者,对照组采用常规护理,观察组在常规护理的基础上采用集束化护理干预,患者转出EICU时,比较2组吸入性肺炎、腹泻、堵管、意外拔管、糖代谢异常的发生率;2组患者血清学指标如血清总蛋白、清蛋白、前白蛋白、血红蛋白及住EICU时间和住院费用进行比较。结果急性重症胰腺炎早期经鼻肠管肠内营养进行集束化护理干预后,观察组患者吸入性肺炎、腹泻、堵管、意外拔管、糖代谢异常低于对照组(P<0.05);观察组患者平均住院时间及住院费用低于对照组(P<0.05),血清学指标优于对照组(P<0.05)。结论集束化护理干预减少急性重症胰腺炎经鼻肠管进行肠内营养的并发症,改善血清学指标,缩短住院时间,减轻患者的经济负担。Objective Toinvestigate therole of clustered nursing intervention to thepatients with severe acute pancreatitis supported by enteral nutrition. Methods A total of 190 patients with severe acute pancreatitis in emergency intensive care unit（EICU） were collected and divided into two groups（95 patients each） ：observation group and control groupran- domly. The observation group received cluster nursing intervention, while the control group received usual care. The rates of inhalation pneumonia, enterorrhea, blocking andunplanned extubation, and abnormal glucose metabolism; the levels of serum total protein, albumin, prealbumin and hemoglobin, average length of stay in ICU, and hospital costs were compared between the two groups. Results When compared with the control group, the rate of aspiration pneumonia, enterorrhea, blocking, accidental extubation and abnormal glucose metabolism were significantly lower in observation group （P 〈 0.05） ; the serological indexes were improved obviously than the control group （ P 〈 0.05 ）. Conclusion The cluster nursing interventions can reduce the complications of enteral nutritional support, improve the serological indexes, shorten the hospital stay and reduce the financial burden of the patients.
