摘  要:目的了解高渗氯化钠羟乙基淀粉40注射液(简称高渗盐复合液,HSH)对全身麻醉诱导期血流动力学的影响。方法 60例择期手术患者随机分为高渗盐复合液组(HSH组)和林格液组(RL组),每组30例。两组患者诱导前30 min内分别输入相应的液体8 m L/kg,然后常规诱导麻醉行气管插管。记录两组患者输液前、输液后、气管插管时、气管插管后5 min,气管插管后20 min的平均动脉压(MAP)、中心静脉压(CVP)、心率(HR)等,记录两组患者输液前、输液后30min、输液后60 min的血钠值。结果输液前两组患者的MAP、CVP、HR差异均统计学意义(P>0.05);输液后HSH组的MAP、CVP比RL组的显著升高(P<0.01)。诱导后RL组MAP、CVP下降明显(P<0.01),HSH组输液后的MAP、CVP、HR变化差异没有统计学意义(P>0.05)。HSH组患者输液后30、60 min的血钠差异无统计学意义(P>0.05)。结论相对林格液,高渗氯化钠羟乙基淀粉40防治全身麻醉诱导期低血压的效果明显,可使患者的血流动力学更稳定。Objective To investigate the effect of Hypertonic sodium chloride hydroxyethyl starch 40(HSH 40) on haemodynamics during induction of general anesthesia. Methods Sixty patients undergoing elective surgery were divided into two groups randomly: HSH group and Ringer's solution(RS) group, with thirty patients in each group. The patients in both groups were administered with HSH(8ml/kg) and RS(8ml/kg) respectively. Then the patients were catheterized after routine anesthesia induction. The MAP, CVP, HR before the infusion, after the infusion, 5min after tracheal intubation, 20 min after tracheal catheterization were recorded. The serum sodium level before the infusion, 30 min and 60 min after the infusion were recorded.Results The MAP, CVP, HR before the infusion were no significant difference in both groups(P〈0.05); The MAP, CVP in HSH group after the infusion were higher than those in RS group(P〈0.05); The MAP,CVP in RS group after catheterization were lower than those before catheterization(P〉0.05), The MAP, CVP in RS group after catheterization were not significant different from those before catheterization(P〉0.05); The serum sodium level 30 min after the infusion, 60 min after the infusion were slightly higher than it before the infusion. But it was no statistical significance(P〉0.05). Conclusion HSH is superior to RS in controlling the hypotension after catheterization.
