人冻干重组脑钠肽联合米力农及多巴酚丁胺治疗难治性心力衰竭的临床观察

摘　　要：目的观察人冻干重组脑钠肽联合米力农及多巴酚丁胺治疗难治性心力衰竭的疗效。方法选取难治性心力衰竭患者51例,随机分为A组(联合用药组)、B组(人冻干重组脑钠肽联合米力农组)、C组(人冻干重组脑钠肽联合多巴酚丁胺组),每组17例,比较3组的临床疗效、安全性、血流动力学改变、血浆脑钠肽等指标。结果 A组的疗效明显高于B组和C组;与治疗前比较,3组的血流动力学改变、血浆脑钠肽均有改善,且A组效果最好,差异有统计学意义(P<0.05或0.01);3组不良反应比较差异无统计学意义(P>0.05)。结论人冻干重组脑钠肽联合米力农及多巴酚丁胺治疗难治性心力衰竭安全有效。Objective To observe the curative effect of lyophilized recombinant human brain natriuretic peptide combined with milrinone and dobutamine on refractory heart failure. Methods 51 patients with refractory heart failure were randomly divided into Group A（n=17）, Group B（n=17） and Group C（n=17）. Group A was treated with lyophilized recombinant human brain natriuretic peptide combined with milrinone and dobutamine. Group B was treated with lyophilized recombinant human brain natriuretic peptide combined with milrinone. Group C was treated with lyophilized recombinant human brain natriuretic peptide combined with dobutamine. Three groups were compared in terms of curative effect, safety, LVEF and plasma BNP level. Results Group A had a curative effect significantly higher than Group B and C; Compared with that before treatment, three groups were all improved in terms of LVEF and plasma BNP level, and Group A achieved the best results, and the difference was statistically significant（P〈0.05 or 0.01）; There was no statistical difference among three groups in terms of adverse reactions（P〉0.05）. Conclusion It is effective and safe to treat refractory heart failure with lyophilized recombinant human brain natriuretic peptide combined with milrinone and dobutamine.
