On the resistance of Acinetobacter baumannii in different clinical departments

摘  要:目的通过分析鲍曼不动杆菌(Acinetobacter baumannii,AB)的耐药性和在临床科室中的分布特点,为控制耐药鲍曼不动杆菌播散和合理利用抗生素提供依据。方法 2012年4月-2014年6月间从临床分离非重复AB 162株,采用全自动微生物分析仪(BDPHOENIX100)进行细菌鉴定和药物敏感性试验,使用卡方检验分析相关数据。结果分离出162株AB中氨苄西林的耐药率最高(100.0%),多粘菌素的耐药率最低(1.9%),其他各种抗生素都有不同程度的耐药;科室分布中ICU的AB最常见(43.2%),其次是血液内科(19.1%);各类抗生素的耐药率在ICU最高,而且头孢哌酮/舒巴坦等的耐药率在ICU中明显高于其他科室(P<0.05),美罗培南和复方磺胺的耐药率在血液内科明显高于除ICU外的其他科室(P<0.05)。结论 AB的耐药形势严峻,其多重耐药趋势明显,特别是在ICU。Objective To investigate drug resistance and distribution characteristics in clinical departments of Acinetobacter baumannii(AB), so as to provide evidence for the control of the dissemination of drug-resistant AB and reasonable clinical use of antibiotics.Methods 162 strains of non-repetitive AB were separated between April 2012 and June 2014. Bacteria identification and drug susceptibility test were carried out with automatic microorganism analyzer(BDPHOENIX100). And the data were analyzed with chi square test. Results Of 162 AB strains separated, the resistance rate to ampicillin was the highest, the resistance rate to polymyxin was the lowest(1.9%), and there was resistance of different levels to other antibiotics. As for their distribution in departments, ICU was the most common department for AB(43.2%), followed by Department of Hematology(19.1%). The resistance rate to various antibiotics in ICU was the highest, the resistance rate to cefoperazone/tazobactam in ICU was significantly higher than that in other departments(P〈0.05), and the resistance rate to meropenem and cotrimoxazole in Department of Hematology was significantly higher than that in other departments except for ICU(P〈0.05). Conclusion There is a serious situation for the drug resistance of AB, and there is obvious trend of multi-drug resistance, especially in ICU.
