摘　　要：目的观察健康管理和疾病管理综合防治社区慢性病效果。方法 100例慢性病患者给予健康管理和疾病管理的综合防治,比较综合防治后100例患者的病症改善情况。结果干预后100例慢性病患者血压,血糖得到了有效的控制,且患者治疗依从性和对病症知识的知晓率也获得了上升。结论健康管理和疾病管理综合防治社区慢性病效果明显。Objective To observe the effect of health management and disease management comprehensive prevention for chronic diseases in communities. MethodsA total of 100 patients with chronic diseasewere given health management and disease management comprehensive prevention,were compared the improvement of the symptoms after comprehensive prevention.ResultsThe blood pressure and blood glucose of 100 patients with chronic disease were effectively controlled after intervention,the patient＆#39;s treatment compliance and awareness of disease knowledge were also significantly increased.Conclusion Health management and disease management comprehensive prevention for chronic diseases in communities can obtain the obvious effect.
