抗菌药物与切开挂线术治疗对肛肠疾病肛周菌群影响的研究

摘　　要：目的探讨抗菌药物与切开管线术治疗肛肠疾病对肛周菌群的影响,为肛周脓肿寻找良好的治疗方法。方法选取2011年3月-2014年1月肛周脓肿患者81例,随机分为对照组39例,实施常规抗菌药物治疗;观察组42例在对照组基础上联合切开挂线术治疗,治疗一周后,对两组患者治疗前后菌群数量、血浆内毒素水平进行比较与治疗效果进行评价。结果观察组与对照组患者大肠埃希菌和双歧杆菌菌落数均较治疗前明显降低,且观察组患者降低程度高于对照组,差异有统计学意义(P<0.05);治疗后对照组患者真菌菌落数与治疗前比较差异无统计学意义,观察组患者真菌菌落数与治疗前明显降低,差异有统计学意义;治疗后观察组患者内毒素水平明显低于对照组,差异有统计学意义(P<0.05);观察组患者痊愈率、有效率明显高于对照组,差异有统计学意义。结论切开挂线术配合抗菌药物治疗肛肠疾病可以更为明显的改善患者肛周病原菌数量,有效避免血浆内毒素水平升高,明显改善患者的预后。OBJECTIVE To investigate the influence of antibiotics and incision and thread‐drawing treatment on ano‐rectal disease and perianal flora so as to find effective treatment for perianal abscess .METHODS Totally 81 cases of patients with perianal abscess were randomly divided into two groups from Mar .2011 to Jan .2014 ,making it comparable .The control group with 39 cases was treated with routine antibiotic therapy ;while the observation group with 42 cases was treated with incision and thread‐drawing on the basis of the control group .After a week＇s treatment ,the numbers of flora and plasma endotoxin levels of the two groups were compared before and after treatmentaswellasthetreatmenteffects.RESULTSE.coliandBifidobacteriumcoloniesallreducedsignificantly in the two groups and the observation group had a lower reduction （P〈0 .05） .The flora after treatment in the control group was almost the same and the difference was not significant .The flora in the observation group re‐duced obviously after the treatment and the difference was significant .Endotoxin level in the observation group was significantly less than the control group after the treatment （P〈0 .05） .The recovery rate and effective rate in the observation group was significantly higher than the control group（P〈 0 .05） .CONCLUSION Incision and thread‐drawing cooperated with antimicrobial treatment have a obvious effect on curing anorectal disease and re‐ducing perianal pathogens and this can effectively avoid elevated plasma endotoxin levels to obtain a better progno‐sis .
