替考拉宁治疗导尿管相关性泌尿道感染疗效与安全性分析

摘　　要：目的评价国产替考拉宁治疗革兰阳性球菌所致的导尿管相关性泌尿道感染(CAUTI)的疗效与安全性。方法调查2012年1月-2014年1月ICU127例应用替考拉宁治疗的革兰阳性球菌所致导尿管相关性泌尿道感染患者临床资料,对其进行治疗前后的临床症状、体征、临床疗效、细菌清除及不良反应分析,并对临床分离菌进行替考拉宁体外药敏试验。结果替考拉宁治疗革兰阳性球菌所致导尿管相关性泌尿道感染,痊愈率40.16%、显效率43.31%、进步率11.81%、无效率4.72%,临床总有效率为83.46%;治疗后细菌清除率为87.25%,其中粪肠球菌和鸟肠球菌的清除率更高,不良反应发生率为3.94%。结论替考拉宁治疗导尿管相关性泌尿道感染疗效显著,安全可靠,不良反应小。OBJECTIVE To evaluate the efficacy and safety of domestic teicoplanin in treatment of the catheter‐asso‐ciated urinary tract infections （CAUTI） due to gram‐positive cocci .METHODS The clinical data were collected from the 127 ICU patients with CAUTI induced by gram‐positive cocci who were treated with teicoplanin from Jan 2012 to Jan 2014 ,then the clinical symptoms ,signs ,clinical efficacy ,bacterial clearance ,and adverse reactions were observed before and after the treatment ,and the in vitro drug susceptibility testing of teicoplanin was per‐formed for the clinical isolates .RESULTS After the patients with CAUTI induced by gram‐positive cocci were trea‐ted with teicoplanin ,the cure rate was 40 .16% ,the markedly effective rate 43 .31% ,the progression rate 11 . 81% ,the ineffective rate 4 .72% ,the total effective rate of clinical treatment 83 .46% ,the bacterial clearance rate 87 .25% ,with the clearance rates of Enterococcus f aecalis and Enterococcus avium even higher ;the incidence of adverse reactions was 3 .94% .CONCLUSION Teicoplanin can achieve significant efficacy in treatment of CAUTI , and it is safe and reliable ,with the incidence of adverse reactions low .
