纤维支气管镜肺泡灌洗吸痰术治疗ICU患者肺部感染临床效果分析

摘　　要：目的探讨纤维支气管镜肺泡灌洗吸痰术治疗ICU患者肺部感染的临床效果,以提高疗效率。方法将2012年11月-2013年10月84例ICU肺部感染患者分为两组,对照组41例给予常规治疗,试验组43例于常规治疗基础上给予纤维支气管镜肺泡灌洗吸痰术治疗。结果试验组、对照组总有效率分别为93.02%、68.29%,与对照组相比,试验组总有效率较高,肺部啰音缓解时间、发热缓解时间、咳嗽缓解时间、抗菌药物应用时间、机械通气时间及ICU住院时间均比较短,两组差异有统计学意义(P<0.05)。结论纤维支气管镜肺泡灌洗吸痰术治疗ICU患者肺部感染,疗效高,症状缓解时间短,药物应用、机械通气及ICU住院时间较短,值得推广。OBJECTIVE To study clinical effect of bronchoalveolar lavage and sputum intubation using fiberoptic bronchoscopy in the treatment of lung infections in ICU so as to improve curative effect .METHODS Totally 84 ca‐ses of ICU pulmonary infection patients were collected from Nov .2012 to Oct .2013 and divided into two groups：41 cases in the control group ,given conventional treatment ,and 43 cases in the experimental group received bron‐choalveolar lavage and sputum intubation using fiberoptic bronchoscopy on the basis of conventional treatment . RESULTS The total effective rates in the experimental group and control group were 93 .02% and 68 .29% respec‐tively .Compared with the control group ,the experimental group was higher in total effective rate ,and shorter in the relief time of lung rale ,fever and cough ,also shorten in antibiotic use time ,mechanical ventilation time and ICU hospitalization time . The differences between the two groups had statistical significance （ P〈 0 .05 ） . CONCLUSION Fiberoptic bronchoscopy used in bronchoalveolar lavage and sputum intubation has high curative effect ,short symptom relief time ,drug application time ,mechanical ventilation time and ICU hospitalization time , hence is worth promoting .
