摘 要:目的了解东营市布病疫情流行特征及其影响因素,为制定防控策略提供依据。方法采取被动监测和主动监测的方法发现病例,采用血清试管凝集试验方法确诊病例,对高发区实施综合干预措施。结果 2006-2013年东营市报告布病病例300例,年均发病率为1.81/10万;疫区从2006年的1个乡镇扩大到2013年5个县区25个乡镇;2006-2009年病例以家畜屠宰及畜产品加工职业人群为主,无明显季节性;2010-2013年病例以饲养职业人群为主,在3-6月份呈现明显季节高峰;采取综合干预措施的地区病例"发病-诊断"时间较其他地区明显缩短。结论东营市布病发病职业由屠宰加工转为饲养,疫情呈上升趋势,疫区扩大的原因与动物检疫、淘汰措施不到位有关,实施人群综合干预措施可使病人在发病早期及时得到治疗。Objective The aim of this study was to understand the epidemiological characteristics and the influencing factors of brucellosis in Dongying, in order to evaluate the efficacy of the intervention measures. Methods Passive and active surveillance was used to identify the eases. Serum agglutination test was used for laboratory diagnosis. All the laboratory confirmed cases were investigated. Intervention measures were used in high-incidence counties. Descriptive epidemiologieal approaches were employed to analyze the surveillance data. Results A total of 300 eases were reported during this period. The average annual incidence rate was 1.81/lakh. Infected areas were spreading from one town in 2006 to 25 towns in 2013. Most cases from 2006 to 2009 were personnel performing livestock slaughtering and related products processing. The disease occurred with no obvious seasonal feature. Most eases from 2010 to 2013 were personnel engaged in breeding. The disease mainly occurred from March to June. The interval between onset and diagnosis in counties using intervention measures was shortened for 14 days than in those not using interventions. Conclusion The career of eases in Dongying changed from livestock slaughtering and related products processing to breeding. The main reason of cases increasing and infected areas expanding was that animal quarantine and elimination measures were not in place. App]ication of intervention measures can shorten the interval between onset and diagnosis.
