摘 要:目的研究临床分离的28株产ESBL多重耐药大肠埃希菌对抗生素的耐药情况和对3种消毒剂的MICs与MBCs,分析其耐药性与消毒剂抗力的关系。方法收集某医院临床分离出的产ESBL多重耐药大肠埃希菌,进行药敏试验,并测定其对氯己定、苯扎溴铵、2,4,4-三氯-2-羟基二苯醚的MICs和MBCs,并与大肠埃希菌ATCC25922相比较。结果 28株试验菌呈现25种耐药谱,有21种抗生素出现耐药现象。分别有35.7%、89.3%、96.4%的菌株对氯己定、苯扎溴铵、2,4,4-三氯-2-羟基二苯醚的MICs和MBCs均高于标准株。结论临床分离的产ESBL多重耐药大肠埃希菌对抗生素有较高耐药性,同时亦对苯扎溴铵和2,4,4-三氯-2-羟基二苯醚呈现出MIC和MBC升高;各种来源菌株的耐药性与消毒剂抗性无明显相关性。Objective This paper was aimed to investigate the antibiotic and disinfectant resistance level of ESBL-producing multiple drug-resistant E.coli in clinical isolation strains, and analyze the relationship between antibiotic resistance and disinfectant resistance. Methods Clinical isolates of ESBL-preducing multiple drug-resistant E.coli were collected from a hospital. And the antibiotic susceptibility tests were conducted with VITEK II. The resistance level of clinical isolates against disinfectants was determined via minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) tests. Results The tested 28 ESBL-preducing multiple drug-resistance E.coli isolates showed resistance against 21 antibiotics, and produced 25 resistance patterns. And the percentages of strains with high MICs and MBCs against chlorhexidine, benzalkonium, triclosan than reference E.coli strain ATCC 25922 were 35.7%, 89.3%, and 96.4% respectively. Conclusion The ESBL-producing multiple drug-resistant E.coli showed wide antibiotic resistance, and higher resistance against benzalkonium and triclosan. There was no significant correlation between the antibiotic resistance patterns and disinfectant resistance.
