经导管射频消融术对不同类型心房颤动患者的疗效观察

摘　　要：目的研究不同类型的心房颤动患者经导管消融术治疗的临床疗效及安全性。方法连续选择2009年1月至2012年1月复旦大学附属中山医院青浦分院收治,诊断为心房颤动且行经导管射频消融术治疗的患者共86例,其中阵发性心房颤动组48例,非阵发性心房颤动组(包括持续性心房颤动和永久性心房颤动)38例。比较两组单次消融的成功率及并发症情况的差异。结果阵发性心房颤动组的单次消融成功率明显高于非阵发性心房颤动组,差异有统计学意义[89.6%(43/48)vs.57.9%(28/38),P<0.05];阵发性心房颤动组并发症发生率低于非阵发性心房颤动组,差异有统计学意义[4.2%(2/48)vs.18.4%(7/38),χ2=22.596,P=0.000]。结论经导管射频消融术是治疗阵发性心房颤动的有效手段,其单次消融有效率及安全性均优于非阵发性心房颤动。Objectives To compare the efficacy and safety of radiofrequency catheter ablation for different types of atrial fibrillation. Methods Totally 86 consecutive patients with atrial fibrillation undergoing radiofrequency catheter ablation were recruited from January 2009 to January 2012 in Qingpu Medical Center, Zhongshan Hospital, Fudan University.All the patients were divided into paroxysmal atrial fibrillation group（n=48） and non-paroxysmal atrial fibrillation group（n=38, including persistent atrial fibrillation and permanent atrial fibrillation）. Differences in single ablation successful rate and complications between the two groups were compared. Results Single ablation successful rate in paroxysmal atrial fibrillation group was significantly higher than that in non-paroxysmal atrial fibrillation group [89.6%（43 / 48） vs.57.9%（28 / 38）,P〈0.05 ]. Complications in paroxysmal atrial fibrillation group was significantly fewer than that in nonparoxysmal atrial fibrillation group [4.2%（2 / 48）vs.18.4%（7 / 38）,χ2=22.596,P=0.000]. Conclusions Radiofrequency catheter ablation is an effective approach in patients with paroxysmal atrial fibrillation. The single ablation successful rate and safety rating are prior in patients with paroxysmal atrial fibrillation.
