摘  要:目的评价舒芬太尼后处理对大鼠心肌缺血再灌注损伤肌钙蛋白的影响,并探讨其可能的机制。方法雄性SD(Sprague-Dawley)大鼠30只,采用随机数字表法将大鼠随机分为5组(n=6):假手术组(S组)、缺血再灌注组(I/R组)、舒芬太尼低剂量组(SL组)、舒芬太尼高剂量组(SH组)和舒芬太尼+磷酸酰肌醇3激酶(phosphatidylinositol 3-kinase,PI3K)抑制剂渥曼青霉素组(WM组)。采用结扎左冠状动脉前降支30 min后再灌注120 min的方法制备心肌缺血再灌注模型。于再灌注前10 min及3 min时,各组分别经颈静脉注入以下不同药物:SL组分别注射0.9%氯化钠溶液及舒芬太尼1μg/kg,SH组分别注射0.9%氯化钠溶液及舒芬太尼3μg/kg,WM组分别注射渥曼青霉素15μg/kg及舒芬太尼1μg/kg,S组及I/R组注射相同容量的0.9%氯化钠溶液。于平衡后、缺血30 min、再灌注30 min、再灌注60 min、再灌注120 min时记录平均动脉压(MAP)和心率。于再灌注120 min时采集大鼠颈动脉血样,采用酶联免疫吸附试验(ELISA)法检测血浆心肌肌钙蛋白I(cardiac troponins I,c Tn I)浓度。结果各组心率、平均动脉压比较,差异无统计学意义(P>0.05)。与S组比较,I/R组、SL组、SH组、WM组血浆c Tn I浓度均明显升高,差异有统计学意义(P<0.01)。与I/R组比较,SL组、SH组、WM组血浆c Tn I浓度均明显降低,差异有统计学意义(P<0.01)。与SL组比较,SH组血浆c Tn I浓度明显降低,差异有统计学意义(P<0.01);WM组血浆c Tn I浓度明显升高,差异有统计学意义(P<0.01)。结论舒芬太尼后处理可减轻大鼠心肌缺血再灌注损伤,该保护作用与PI3K相关信号通路的激活有关。Objectives To evaluate the effect of sufentanil postconditioning on cardiac troponins I(c Tn I) during myocardial ischemia-reperfusion(I / R) injury in rats, and to investigate the possible mechanism. Methods Thirty male Sprague-Dawley rats were randomly divided into 5 groups(n=6 each) : sham group(S group), I / R group, lowdose sufentanil postconditioning group(SL group), high-dose sufentanil postconditioning group(SH group) and sufentanil postconditioning plus phosphatidylinositol 3-kinase(PI3K) inhibitor wortmannin group(WM group).Myocardial I / R model was produced by occlusion of left anterior descending artery for 30 min followed by reperfusion for 120 min. Different drugs were injected via the jugular vein 10 min and 3 min before reperfusion respectively : normal saline and sufentanil 1 μg / kg in SL group, normal saline and sufentanil 3 μg / kg in SH group, wortmannin 15 μg / kg and sufentanil 1 μg / kg in WM group, and equal volumes of normal saline in S group and I / R group. Mean arterial pressure(MAP) and heart rate were recorded at complete equilibration, 30 min of ischemia, 30 min, 60 min and 120 min of reperfusion. After 120 min of reperfusion, artery blood was collected for assessment of plasma concentration of c Tn I.Results There were no differences in MAP and heart rate among all the groups(P〉0.05). Compared with S group,plasma concentrations of c Tn I significantly increased in the rest four groups(P〈0.01). Compared with I / R group,plasma concentrations of c Tn I significantly decreased in SL group, SH group and WM group(P〈0.01). Compared with SL group, plasma concentration of c Tn I significantly decreased in SH group(P〈0.01), but significantly increased in WM group(P〈0.01). Conclusions Sufentanil postconditioning can attenuate myocardial I / R injury in rats. The mechanism involves PI3 K pathways.
