PET/CT联合HRCT在诊断孤立性肺结节中的应用价值

摘　　要：目的探讨PET/CT联合HRCT在诊断孤立性肺结节(solitary pulmonary nodule,SPN)中的应用价值,为其推广应用提供依据。方法采用回顾性分析,270例患者均行PET/CT显像和病灶部位HRCT检查,将影像诊断结果与病理诊断结果相对照,并进行统计学分析。结果 270例SPN患者中,恶性病变200例,良性病变70例。单纯PET/CT、单纯HRCT及二者联合诊断SPN良恶性病变的灵敏度、特异性、准确性分别为96%、65.7%、88.1%,91.5%、61.4%、83.7%,97.5%、82.9%、93.7%。PET/CT和HRCT单独诊断SPN的准确性之间无统计学差异(P>0.05),二者联合诊断SPN的准确性明显优于单独应用(P<0.01)。结论 PET/CT显像联合HRCT诊断SPN良恶性的准确性优于单独应用,具有灵敏、特异及定位精确等特点,可达到早期发现病灶和诊断疾病的目的。Objective To explore the application of ^18F-fluorodeoxyglucose （^18F-FDG） positron emission tomograhy/computed tomography （PET/CT） combined with high-resolution computed tomography （HRCT） to the diagnosis of solitary pulmonary nodule （ SPN）, so as to provide the evidence for its widespread clinical use. Methods A total of 270 patients all received the ^18F-FDG PET/CT and HRCT examination in the mean-time were retro- spectively reviewed ; the results of imaging diagnosis were compared with those of pathologic diagnosis, and the statistical analysis was performed. Results Among 270 SPN cases, 200 showed malignant lesions; and 70 benign lesions. The sensitivity, specificity and accuracy of diagnosing SPN with PET/CT alone were 96.0%, 65.7%, 88.1% respectively; those with HRCT alone were 91.5%, 61.4%, 83.7% respectively; and those with PET/CT ＋ HRCT were 97.5%, 82.9%, 93.7% respectively. There was no statistical difference in accuracy with PET/CT alone, or HRCT alone to diagnose the SPN （ P〉0.05 ）. But the accuracy of PET/CT combined with HRCT was higher than PET/CT alone, or HRCT alone（P〈0.01 ）. Conclusions lSF-FDG PET/CT combined with HRCT is an effective and noninvasive way to discriminate malignant SPN from benign one. It has higher sensitivity, accuracy, and speci- ficity for early diagnosis and early treatment.
