摘 要:目的了解郴州市2010—2013年手足口病聚集性病例的流行病学特征,为制定防控措施提供依据。方法采用描述性流行病学方法对郴州市2010—2013年手足口病聚集性病例监测数据进行分析。结果2010—2013年郴州市共报告手足口病聚集性病例疫情80起,病例数256例。疫情主要发生在家庭(67.5%,54/80),报告病例数以2--5例为主(87.5%,70/80)。报告发生时间主要集中在3—6月(85%,68/80),呈明显季节性。病例以3岁及以下人群为主(83.2%,213/256),男性多于女性。病原学监测提示,2010年和2012年聚集性病例以Ev71感染为主,2011年以CoxA16感染为主,2013年以其他肠道病毒感染为主。结论2010—2013年郴州市手足口病聚集性病例的流行特征同当年手足口病全部病例的流行特征一致。引起手足口病的人肠道病毒都可以引起聚集性病例的发生,引起聚集性病例的病原体型别与流行的优势毒株有关。Objective To investigate the epidemiological characteristics of cluster cases of hand - foot - mouth disease (HFMD) in Chenzhou City from 2010 to 2013 so as to provide evidence for developing its prevention and control measures. Methods Descriptive epidemiological method was used to analyze the monitoring data about cluster cases of HFMD in Chenzbou City from 2010 to 2013, and chi - square test was employed to compare corresponding rates and ratios. Results A total of 256 HFMD cluster cases and 80 outbreaks were reported in Chenzhou City during 2010 - 2013. The outbreaks occurred mainly in the family (67.5%, 54/80), and the number of reported cases in a single outbreak was from 2 to 5 (87.5%, 70/80). Most cluster cases were reported from March to June (85 %, 68/80), which showed obvious seasonal variation. Most cluster cases were children aged between 0 and 3 years (83.2%, 213/256), and male cases were more than female ones. Etiological monitoring in- dicated that EVT1 was the main pathogen in 2010 and 2012, CoxA16 was the main pathogen in 2011 and the other human en- teroviruses were the main pathogens in 2013. Conclusions The epidemiological characteristics of HFMD cluster cases were similar to those of all HFMD cases in Chenzhou City from 2010 to 2013. All the human enterovimses which cause HFMD can cause HFMD cluster cases, and the pathogens which cause the cluster cases are associated with the prevalent superior strains.
