低分子量肝素联合硫酸镁治疗早发型子痫前期的疗效观察

摘　　要：目的探讨早发型子痫前期实施低分子量肝素联合硫酸镁治疗的临床效果。方法入选符合标准的早发型子痫前期患者116例,按治疗方式的不同分为治疗组和对照组,每组58例。治疗组给予低分子量肝素联合硫酸镁治疗,对照组给予常规硫酸镁治疗,观察两组治疗前后肾功能、凝血功能、并发症、血压及妊娠延长天数等各项指标。结果两组治疗后,血压和凝血功能各项指标均有变化,但治疗组血清D-二聚体指标值显著优于对照组,治疗组24h尿蛋白、尿蛋白/尿肌酐指标、妊娠天数、新生儿重度窒息发生率等显著优于对照组,具有统计学差异(P<0.05);而PT、FBG、APTT等其他指标、并发症发生率等相比较,差异无统计学意义(P>0.05)。结论低分子量肝素联合硫酸镁治疗早发型子痫前期,有效地改善了患者的肝肾功能与高凝血状态,疗效显著,值得临床推广。Objective To explore the curative effect of low molecular weight heparin combined with magnesium sulfate on early-onset preeclampsia.Methods 116 patients with early-onset preeclampsi- a in our hospital were selected and divided into the treatment group and the control group ac- cording to different treatment methods,with 58 patients in each group. The treatment group was treated with low molecular weight heparin combined with magnesium sulfate, and the control group was treated with only magnesium sulfate. The indexes of renal function, blood coagulation function, complications, blood pressure and gravidity extension days of both groups were observed before and after the treatment. Results The indexes of blood pressure and blood coagulation function of both groups changed after treatment. The index values of serum D-dimer, 2＂4h urine protein, index of urine protein/urine creatinine, gravidity extension days and the incidence of neonatal severe as- phyxia of the treatment group were significantly better than those of the control group, and the results were statistically significant （P〈0.05）. The differences in other indexes such as PT, FBG, APTI＂ and the incidence of complications were not statistically significant （P〉0.05）. Conclusions Low molecular weight heparin combined with magnesium sulfate has good effect in treating early- onset preeclampsia, and can effectively improve the kidney function and high blood coagulation of patients. It has distinct clinical curative effect and thus is worth clinical promotion.
