解毒化瘀疏肝方对慢性乙型病毒性肝炎外周血T淋巴细胞亚群的影响

摘　　要：目的:探讨自拟解毒化瘀疏肝方对慢性乙型病毒性肝炎(CHB)患者T淋巴细胞亚群的影响。方法:将86例CHB患者随机分为两组,每组43例,对照组给予阿德福韦酯治疗,观察组在对照组治疗基础上加用自拟解毒化瘀疏肝方药,持续治疗6个月;治疗前、治疗3个月、6个月后均检测外周血T细胞亚群,治疗6个月后观察中医症候积分、乙型肝炎病毒核酸(HBV-DNA)变化。结果:3个月后两组CD4+、CD8+、CD4+/CD8+比较差异无统计学意义(P>0.05);6个月后观察组CD4+、CD4+/CD8+高于对照组(P<0.05);3个月、6个月后观察组中医症状评分低于对照组,差异比较有统计学意义(P<0.05);两组治疗前、后HBV-DNA载量明显下降,两组6个月后HBV-DNA载量比较差异有统计学意义(P<0.01)。结论:对CHB患者加用解毒化瘀疏肝方,可缓解临床症状,抑制HBV-DNA复制,其机理可能与改善T淋巴细胞亚群水平有关。Objective ： To investigate self - made Detoxifying, Removing Blood and Soothing Liver Stasis Formula impact on peripheral blood T lymphocyte subsets of CHB. Methods ：86 patients with CHB were randomly divided into two groups. 43 cases in the control group received adefovir dipivoxil treatment and observation group were treated with self - made pre= scription, continued treatment 6 months;pre -treatment, three and six months later, we detected peripheral blood T cell subsets, symptom scores, hepatitis B virus DNA （ HBV - DNA） changes. Results ： The two groups＇ CIM ＋ , CD8 ＋ , CIM ＋ / CD8 ＋ had no difference three months after treatment （ P 〉 0. 05 ） ; 6 months later, the observation group＇s CIM ＋ , CD4 ＋ / CD8＋ were higher than that of control group （P 〈 0.05 ） ; three and six months later, the observation group＇s TCM symp- tom scores were lower than the control group ＇ s, the difference was statistically significant （ P 〈 0. 05 ） ; before and after treatment,the HBV- DNA load decreased significantly in both groups six months after treatment and HBV -DNA load was statistically significant （ P 〈 0.01 ）. Conclusion： Detoxifying, Removing Blood and Soothing Liver Stasis Formlua; can relieve symptoms and inhibit HBV - DNA replication of CHB patierts. The mechanism may be related to improving the level of T lymphocyte subsets.
