肺心标2号治疗慢性肺心病急性加重期热痰证的疗效观察

摘　　要：目的:观察肺心标2号治疗慢性肺源性心脏病急性加重期热痰证的临床疗效。方法:将72例符合纳入标准的患者随机分为治疗组和对照组(各36例)。对照组给予西医常规治疗,治疗组在西医常规治疗基础上加用肺心标2号,疗程为1周。观察两组治疗前后临床症状、体征,检测C反应蛋白、PCT、血气分析指标。结果:1治疗组愈显率高于对照组,差异具有统计学意义(P<0.05),两组有效率比较差异无统计学意义(P>0.05)。2两组治疗后CRP、PCT较治疗前下降,差异具有统计学意义(P<0.05),治疗后治疗组CRP、PCT水平较对照组降低,差异具有统计学意义(P<0.05)。3血气分析:两组治疗后PH、Sa O2、Pa O2水平均较治疗前上升,Pa CO2水平均较治疗前下降,差异均具有统计学意义(P<0.05);治疗后治疗组较对照组p H、Sa O2、Pa O2水平升高,Pa CO2水平下降,差异均具有统计学意义(P<0.05)。结论:肺心标2号联合西医常规治疗可以更好地改善慢性肺源性心脏病急性加重期热痰证患者的临床症状、体征,减轻炎症反应,改善动脉血液气体分压。Objective ： To observe the clinical efficacy of Feixin NO - 2 Decoction treating patients with chronic pulmo- nary heart diseases in acute exacerbation（ type of heat -phlegm）. Methods ：72 cases of patients according to the diagnosis standards in the study were selected and divided into two groups randomly：the control group 36 cases were given western medicine therapy regularly and the treatment group 56 cases added Feixin NO -2 Decoction on the basis of the western medicine therapy. The changes of clinical symptoms, signs, c - reaction protein, procalcitonin, the arterial blood gas analy- sis were observed before and after treatment in both groups. The treatment course was 2 weeks. Results ： （1）The number of the fully recovered and almostly healed cases in the treatment group was higher than that in the control group, which was statistically significant （P 〈 0.05 ）. The total effective rate tetween the two groups was not statistically significant（ P 〉0. 05）. （2）The levels of CRP and PCT decreased after treatment in both groups （P 〈 0.05）. The levels of CRP and PCT in the treatment group were lower than those in the control group after treatment, which showed significant difference （ P 〈 0.05 ）. （3）The levels of pH, PaO2 and SaO2 increased, and the level of PaC02 decreased after treatment in both groups （P 〈 0.05 ）. After treatment,the levels of PH, PaO2 and SaO2 in the treatment group were higher than those in the control group and the level of PaCO2 in the treatment group was lower than that in the control group, which showed significant difference （ P 〈 0. 05 ）. Conclusion ： Feixin NO - 2 Decoction combined western medicine therapy could effectively alleviate clinical symptoms and signs for patients of chronic pulmonary heart diseases in acute exacerbation （ type of heat - phlegm）, showing significant improvement in arterial blood gaspartial pressure, and lower the levels of inflammatory mark- ers.
