2型糖尿病合并高尿酸血症与心血管疾病危险因素的相关性研究

摘　　要：目的探讨T2DM合并高尿酸血症（HUA）与心血管疾病危险因素的关系。方法回顾性调查T2DM患者3174例,分析336例T2DM合并HUA患者与心血管危险因素的关系。结果T2DM患者3174例,其中,男2441例,女733例,年龄24~91岁,平均（60.23±9.77）岁;T2DM合并HUA（HUA组）患者336例,HUA患病率为10.6%。HUA组年龄≥60岁（65.2%vs 48.1%）、女性（29.3%vs 22.3%）、BMI≥25.0kg/m2（74.4%vs 59.9%）、TG≥1.70mmol/L（59.8%vs 42.4%）、TC≥5.7mmol/L（37.8%vs 29.1%）、HDL-C〈1.04mmol/L（13.7%vs 8.8%）、血肌酐（Scr）≥110μmol/L（30.4%vs 19.0%）、BUN≥7.0mmol/L（41.1%vs 26.2%）的患者比例高于单纯T2DM组（P〈0.05）。二分类非条件Logistic回归分析显示,高龄、女性、BMI≥25.0kg/m2、高TG血症、高TC血症、低HDL-C血症、Scr≥110μmol/L、BUN≥7.0mmol/L是T2DM患者发生HUA的独立危险因素（P〈0.05）。结论T2DM患者合并HUA与高龄、女性、肥胖、血脂异常及肾功能减退相关。Objective To explore the relationship of hyperuricemia（HUA）and cardiovascular risk factors in T2 DM. Methods A total of 3174 subjects with T2 DM were screened.The relationship between HUA and cardiovascular risk factors were evaluated in 336 subjects with HUA. Results A total of 3174T2 DM were enrolled in this study.Male and female were 2441 and 733respectively.The average age was（60.23±9.77）years,from 24 to 91years old.There were 336 patients with HUA.The prevalence rate of HUA was 10.6%in this cohort.The rate of age≥60years（65.2%vs 48.1%）,female（29.3%vs 22.3%）,BMI≥25.0kg/m2（74.4%vs 59.9%）,TG≥1.70mmol/L（59.8% vs 42.4%）,TC≥5.7 mmol/L（37.8% vs 29.1%）,HDL-C〈1.04 mmol/L（13.7% vs 8.8%）,Scr≥110μmol/L（30.4%vs 19.0%）,BUN≥7.0mmol/L（41.1% vs 26.2%）in HUA patients were higher than in nonHUA patients（P〈0.05）.Binary non-conditional logistic regression analysis showed that,advanced age,female,BMI≥25.0kg/m2,high TG,high TC,low HDL-C,Scr≥110μmol/L,BUN≥7.0 mmol/L were independent risk factors for HUA in T2 DM patients（P〈0.05）. Conclusion Advanced age,female,obesity,dyslipidemia and renal impairment are associated with the occurrence of HUA in T2 DM.
