温州市某综合医院2009—2013年HIV抗体初筛与确诊结果分析

摘　　要：目的了解温州市某综合医院患者中HIV感染状况、科室分布以及国家HIV检测策略在地方的实施效果。方法分析2009—2013年就诊患者HIV抗体检测数据的初筛与确诊结果。结果检测476 963例患者,总确诊阳性率为0.071%。5年间确诊阳性率,2009—2010年间上升明显,差异具有统计学意义;2010—2013年间差异无统计学意义。感染者检出科室广泛,主要集中在皮肤科、急诊科、感染内科及呼吸内科等。初筛和确诊总阳性总符合率为61.08%,确诊等待周期平均为(9.71±4.97)天。结论该院患者HIV确诊阳性率2009—2010年呈上升趋势,其后略稳定,但确诊数增多,科室分布广泛。医务人员应提高自我防护意识。建议国家改进检测策略,提高初筛实验灵敏度、特异度,缩短确诊周期,对早诊断、早治疗HIV患者具有重要意义。OBJECTIVE To investigate the positive rate of HIV antibody in patients of a general hospital in Wenzhou and estimate theimplementation of the National HIV detection strategy in Wenzhou. METHODS The plasma HIV antibodies of 476 963 patients weredetected from 2009 to 2013,and the positive cases were analyzed. RESULTS Totally 339 plasma samples were positive by the WBtest for HIV and the positive rate was 0.071%. The HIV-positive cases were distributed in 36 clinical departments including Dermato-logical Department,Emergency Department,Infectious Disease Department,Pneumology Department,ect. Among 555 positivessamples tested by anti-HIV screening test,339 samples were confirmed to be positive by WB test with coincidence rate of 61.08%.The patients needed to wait an average of（9.71±4.97） days for the results of WB test. CONCLUSION The positive rate of HIV anti-body among patients was high. Medical staff should enhance prevention awareness. The health authorities should adjust the strategy toreduce the waiting time of the final results.
