化浊解毒软肝方治疗慢性丙型肝炎肝纤维化的疗效及对血清TNF-α和IFN-γ的影响

摘　　要：目的探讨化浊解毒软肝方治疗慢性丙型肝炎肝纤维化的疗效及对血清肿瘤坏死因子-α(TNF-α)和干扰素-γ(IFN-γ)的影响。方法将60例慢性丙型肝炎肝纤维化患者随机分为2组,对照组30例给予复方鳖甲软肝片口服,治疗组30例给予化浊解毒软肝方口服,2组均服药24周,观察2组治疗前后临床症状评分、肝功能、血清TNF-α和IFN-γ变化。结果 2组治疗后乏力、肝区不适、脘腹胀满及厌食油腻症状评分均较治疗前明显降低(P均<0.05),且治疗组各评分均明显低于对照组(P均<0.05)。2组治疗后ALT、AST、TBil、TNF-α、IFN-γ水平均较治疗前明显降低(P均<0.05),IFN-γ水平较治疗前明显升高(P均<0.05),且治疗组各指标改善情况均明显优于对照组(P均<0.05)。2组治疗后门静脉内径、脾静脉内径、脾厚度均较治疗前有所好转(P均<0.05),且治疗组改善情况优于对照组(P均<0.05)。结论化浊解毒软肝方能有效改善慢性丙型肝炎肝纤维化患者的临床症状体征、肝功能及TNF-α、IFN-γ水平,值得临床推广应用。Objective It is to observed the curative effect of Huazhuo Jiedu Ruangan Decoction （HZJDRGD） on chromic hepatitis C patients with hepatic fibrosis and its influence on serum tumor necrosis factor alpha （TNF - α） and interferon gam- ma （IFN -γ）. Methods 60 eases of chronic hepatitis C patients with hepatic fibrosis were randomly divided into 2 groups with 30 in each. The patients were treated by oral with Compound Biejia Ruangan tablets in the control group and with HZJDRGD in the treatment group, all patients were treated for 24 weeks. The clinical symptoms score, liver function, the levels of serum TNF -α and IFN - γ were observed before and after treatment in 2 groups. Results The score of fatigue, liver area unwell, flatulence, and anorexia greasy after treatment were significantly improved in both groups （ all P 〈 0. 05 ） , and the score of the treatment group were significantly better than those of the control group （ all P 〈 0.05 ）. The levels of ALT, AST, TBil, TNF - α, IFN -γ after treatment were significantly decreased in both groups （all P 〈 0. 05）, and the indexes of the treatment group were significantly better than those of the control group （ all P 〈 0. 05 ）. The diameter of portal vein and splenic vein, the thickness of spleen after treatment were improved than before treatment （ all P 〈 0.05 ）, and the indexes of treatment group were significantly better than those of the control group （ all P 〈 0. 05 ）. Conclusion HZJDRGD can effectively improve the clinical symptoms and signs, liver function and the TNF - α, IFN - γ levels of chromic hepatitis C patients with hepatic fibrosis.
